You’re dealing with a project failure. How can you use it to build your leadership skills?
Before I go into a post-mortem analysis or another similar meeting that could potentially cause a blame session, I make sure to communicate to the team that I don't believe in assigning personal blame. In someone else's shoes we would likely act the same way. Whenever you are tempted to point the finger at another person, pause to consider the bigger picture that led that person to that conclusion. That is likely where the true problem lies. Another suggestion would be to enter the post-mortem after some pre-work. I like to frame the issue (as I understand it) and send the document out to each team member. I ask them to make a copy and share ideas before the meeting. This helps give everyone a voice in the group setting.

I would advise that you look forward to failure as an opportunity to learn for yourself, and to be an example for your team. How you respond will affect how your culture is formed. Accept responsibility quickly. There is no need to be overly dramatic about it, but he honest and up front. Take ownership of your part in it and communicate the importance of learning from this opportunity.
