Policies and Procedures: Enhancing Pharmacy Practice and Limiting Risk
Pharmacy and Therapeutics Journal
P&Ps provide a roadmap for practice, incorporate best practices, and have buy ins from all stakeholders in the institution. They are necessary for practice, especially for high risk therapies, and when not followed, patients have an increased risk of harm. The article provides several case examples of patient injuries resulting in litigation that include discussions about inadequeate P&Ps or deviations of existing P&Ps.

Companion Report Focuses on Medicare Part D and the Medical Home
The 2010-2011 edition of The Boehringer Ingelheim Medicare Part D & Patient-Centered Medical Home Report examines the experiences and opinions of health plans in marketing to Medicare Part D beneficiaries and the extent to which health plans are embracing the medical home concept. Survey findings are analyzed by industry experts and supported by other research.
Among the report findings:
Part D plans are aggressively marketing to younger and healthier patient populations.
The top 5 therapeutic categories in drug spend are: diabetes, hyperlipidemia, hypertension, cancer, and depression.
Nearly one-third of Part D plan members reached the donut hole coverage gap in 2009.
Part D plans agree that widespread adoption of health information technology will improve patient outcomes.
Almost two-thirds of Part D plans support the medical home concept.
