The Criminal Justice Case Manager provides clinical and administrative support to clients with serious psychiatric and/or substance abuse difficulties within the program of Mental Health Partners.

Essential Functions
Assists clients in skill building and resource development in order to successfully live as independently as possible
Assists other teams when necessary with outreach, engagement, and paperwork
Provides a variety of case management services, under supervision, assisting clients to apply for various types of public assistance and providing supporting documentation, attending multi-agency clinical case consultations, and facilitating referrals to other organizations
Supports client recovery by developing and enhancing client access to resources for vocational, financial, medical, housing, utilities, food, transportation or basic living needs as appropriate
Works collaboratively with clients and other pertinent staff to provide assessment, develop treatment plans and coordinate appropriate care
Assists clients in crisis situations to provide appropriate interventions or referrals
Participates in team meetings to staff cases and communicates administrative information
Facilitates groups, classes and workshops as needed
Provides education on daily living skills and encourages client's use of those skills as needed
Provides support in the management of the milieu and the work environment
Face to face (in the office or in a community setting) or phone contact with clients in order to facilitate progress toward recovery through resource development
Effectively interacts and communicates with other MHP staff/clients/customers/partners/etc.

Specialized Duties
Support transition process of clients involved in the criminal justice system to access and engage with MHP services
Works with Boulder County Sheriff’s Dept personnel
Work in the Boulder County Jail, in the community and at MHP sites
Network with inter-agency providers, teams and partners on behalf of client care
Network with external partners to coordinate access to MHP services
Meet with clients in Boulder County Jail to establish rapport, and inform about MHP services and transition plan
Meet with clients at different Mental Health Partners locations/in community to establish rapport, and inform about MHP services and transition plan
Develop, or enhance, external partner relations

What We Need For This Role
Bachelors Degree in Psychology, Social Work, Counseling or related field
Certified Addition Specialist (CAS) or Certified Addition Technician (CAT) | CAC I or II preferred, or working towards it.
