POSITION STATEMENT:

In this role, you will help us achieve our mission to change the way

the world defines and views disabilities by making profound, positive differences in people’s

lives every day. Intake Coordinators work in an Early Intervention Program for children birth-3

years. Easterseals Louisiana is the System Point of Entry (SPOE) that provides intake/ongoing

service coordination for families and children for Region 8 in Louisiana.

Essential Job Responsibilities

  • Assessing referrals received to determine eligibility
  • Facilitating the intake process from referral through eligibility determination, and when appropriate, the Individualized Family Service Plan (IFSP) for Families.
  • Assembling a multidisciplinary team for eligibility determination according to guidelines.
  • Assisting parents in gaining services.

  • Ensuring the services selected to meet the individual families' needs per the assessment process.
  • Providing information to parents for community services such as referrals for hearing or eye exams, WIC, or Families Helping Families.
  • Facilitating the timely delivery of available services. Ensuring that each child is appropriately transitioned at program exit.
  • Create and maintain physical and electronic files and documentation of cases.
  • Maintain contact with families from the time of referral through case culmination using various methods.

  • Coordination - Able to coordinate with various partners effectively to fulfill the intake

process and ensure timely delivery of services. Track and maintain multiple cases at

various stages in the process on an ongoing basis. Be flexible with families and partners and

their changing needs and demands. Able to pivot when necessary while maintaining a good

rapport with team members.

  • Action Orientation - Ability to take initiative, take action, and attempt to influence events to

achieve goals

  • Work Commitment - Willingly to work the time required to get the job done; identify with

organizational goals, exhibit excellent time management abilities

Requirements

QUALIFICATIONS:

A Bachelor's Or Master's Degree In One Of The Following Human Service Fields, From An Accredited Institution In The Areas Of

  • Social work from a program accredited by the Council on Social Work Education
  • Nursing (RN) currently licensed in Louisiana (one year of paid experience will substitute for the degree
  • Other human services related fields, including: psychology, education, counseling, social services, sociology, philosophy, family and consumer sciences, criminal justice, rehabilitation services, child development, substance abuse, gerontology, vocational rehabilitation, and child life and family studies
  • General Studies with a major concentration in either of the above-listed human services fields
  • Liberal arts or general studies with a concentration of at least 16 hours or more in one of the human services-related fields listed above
  • If you have a degree in Social Work you must maintain the required licenses through the state of Louisiana, and continuing education

Physical Requirements

  • The employee is regularly required to operate a computer, file and retrieve written documents, and communicate clearly with others on the telephone and in person.
  • The employee is frequently required to use hands and lift and/or move a minimum of 30 lbs.
  • Must be able to stand, sit, and walk for up to 1 hour intervals.
  • Ability to move independently or with reasonable accommodation within the facility and community.
  • Must be able to travel long distances and meet with participants.
  • Ability to bend, squat, climb, kneel, and twist on an intermittent or continuous basis.
  • Ability to grasp, push, reach, and pull objects such as files, file cabinet drawers, children, and/or other office equipment during an 8-12 hour day.
  • Ability to perform various repetitive motion tasks such as typing and writing.
  • Must maintain focus and keen attention to detail throughout the day.
  • Visual, auditory, and fine motor skills acuity sufficient to evaluate, treat, and monitor client and administrative needs and maintain accurate records.
