Integrated Decision Team Facilitator (Together We Rise)

We are seeking a caring, compassionate, mission driven IDT Facilitator to work specifically with children and their families who have come to the attention of the child protective investigation division of the Pinellas County Sheriff's Office, and who have been determined to be safe but at high risk of future child abuse, and neglect. Together We Rise (TWR) will work with all members of the family with a team based approach in a supportive, empowering and respectful manner to assist the family with creating safe and stable parenting practices to facilitate long-term behavior change that will ensure that children are safe in their home.

Requirements of the Integrated Decision Team (IDT)Facilitator position is to facilitate case conferences on each family to document progress, identify regress, and reduce barriers to family stability. Integrated Decision Team staffing's are held at regular intervals of a minimum of every 15-days. IDT's are built on the principles Multi-Disciplinary Staffing's (MDT), Family Team Conferencing, and High Fidelity Wraparound (HFW). All of these models us a structured, team based, strength based process that are designed to bring all members of the team together to identify the needs of the children and their family, and to keep child safe and well supported. IDT's have added the additional components of "shared risk decision making", and "nothing about me without me" to the essential elements of the case conference. Specifically, all members of the team, including the parent/caregiver must agree, at every staffing, on the areas of risk and the action plan to ensure the safety of the children between IDT staffing's. Upon the family voluntarily agreeing to receive the services of Together We Rise and the assignment to the ICM, the IDT Facilitator will schedule, coordinate and facilitate all necessary IDT staffing's at a minimum of every 15-days starting with the day of intake, and maintain the staffing schedule throughout service provision. The Integrated Decision Team Facilitator will lead the strength based discussion that will identify the needs and resources for the family and will drive the service plan established in partnership with the family to ensure the ICM is facilitating immediate mitigation of risk and the Counselor is addressing the clinical needs of the family (individually and as a family). The Integrated Decision Team Facilitator, operating through the philosophy of "nothing about me without me" coordinates and facilitates the discussion around child safety, behavioral health needs and identifies options, objectives and goals ensuring that the parents are partners in the treatment and service plan who are also identifying issues that need addressed. The IDT Facilitator then facilitates the team to arrive at a shared risk decision based on the best approach with the family to ensure immediate needs are being met, risk of maltreatment has been mitigated and behavior change can be maintained. The IDT Facilitator ensures that the IDT form is projected so that all the parent can see all information being recorded ("nothing about me, without me"), that the IDT take place at times and places convenient to the parent. The position also ensures that the family is in attendance of every staffing, and that all members of the support team to the family are present, to include faith leaders, supportive family members and friends/neighbors. This position is required to complete dual entries into two separate systems: the agency electronic health record and the Florida Safe Families Network (FSFN). DFL prides itself on the fact that it is a trauma informed care services provider. This position requires that trauma informed practices are embedded into all services provided. This position is field based, IDT Staffings are to occur in families homes when possible, at minimum at a mutually identified place, where the family feels most comfortable. Hours are flexible, evening and weekend work is required. This position will work closely with members of the Together We Rise team such as: Counselor, Intensive Case Manager, and TWR Supervisory Team and in collaboration with other community service providers externally to provide complete comprehensive services. This position is responsible to meet the client facing client care requirement of 112 billable hours per month. This is a field based position.

EDUCATION EXPERIENCE/REQUIREMENTS:

  • Bachelor's Degree in Social Work, or related area of study or a master's degree in Social Work, or related area of study from an accredited college or university
  • A minimum of two years of experience working with at-risk populations to include families experiencing substance misuse, domestic violence, mental health, and emotionally disturbed children.
  • Required to become certified as a High Fidelity Wraparound Facilitator within the first six months of hire, and a High Fidelity Wraparound Facilitator COACH within one year of hire.
  • Previous child welfare experience is preferred.
