Key Qualifications
Currently licensed Social Worker in New York
Two years of experience in nursing home surveillance activities
Surveyor Minimum Qualifications Test (SMQT) qualified
Willingness to extensively travel to nursing homes across the five boroughs of New York City
Two years of clinical experience in a medical facility, preferably a nursing home
Proficient in basic computer skills, including Microsoft Office products
Excellent written and verbal communication skills
Role Overview - Surveyor Dietitian/Surveyor Social Worker
As a Surveyor Dietitian/Surveyor Social Worker, you will play a crucial role in assessing and ensuring the adequacy of social and behavioral care provided to nursing home residents. Your responsibilities will include:
Observing and evaluating resident care
Assessing nursing homes' compliance with federal and State regulations
Serving as the nutritional or social services expert for the survey team
Collaborating with clinical staff in nursing facilities to improve existing programs
Participating in investigations related to incidents or complaints regarding nutritional or social services
Contributing to medical reviews and audit mechanisms to enhance services for residents
Engaging in various program-related activities as required
