As the busy winter period for the NHS approaches, we have today urged healthcare leaders to address the lack of confidence that many people have in accessing timely care when they need it. Our new research suggests that almost a third, 32%, of people lack confidence that they can access vital care in a timely way. Our CEO Louise Ansari said: "With demand for care likely to rise this winter, the negative perception that the NHS can’t provide timely care needs urgent attention to ensure people do seek medical attention when needed. “If people are not confident in requesting a referral for treatment or calling an ambulance, they put their health at risk. ***RURAL HEALTH UPDATE - Rural paramedics are making routine house calls to avoid costly emergency room visits*** As we transition to a future of value-based care in healthcare, the importance of preventative care cannot be overstated. A compelling example of this is the innovative community paramedicine programs, which are reshaping healthcare delivery in rural areas. In places like Terlingua, Texas, paramedics are making routine house calls to check on patients with known health issues. This proactive approach helps prevent costly emergency room visits and reduces the strain on healthcare systems. With hospitals and doctors in short supply, community paramedicine is a lifeline for many rural residents. By addressing health issues before they escalate into emergencies, these programs not only improve patient outcomes but also offer significant cost savings. For instance, regular check-ins can reduce hospital readmissions, which cost hospitals millions in penalties annually. However, the economic model for these programs remains challenging. Most insurance providers, including Medicare and Medicaid, do not cover the costs of paramedics making house calls. Sustainable funding is crucial for the long-term success of these programs. We must advocate for policy changes and support from insurers to recognize the value of community paramedicine. Preventative care is essential for a healthier, more sustainable healthcare system. Ambulance transport may be crucial for the transition from hospital to home care. However, the prior authorization model often creates obstacles for ambulance providers. Fortunately, payers are recognizing that this model is not always the best route. Blue Cross Blue Shield of Massachusetts is eliminating more prior authorizations. Behind closed doors, the Commonwealth and State hospital funding agreement (NHRA) is currently under negotiation. When the Agreement is finally inked, it will be interesting to learn if Government has adopted some of the innovations proving so successful in the private hospital system and internationally. In particular, will Government pivot to allocate greater resources towards day hospital facilities - to help efficiently and effectively address very long public elective care waits? Public patients should not have to wait years, in pain, for care. Here is an interesting article from the U.K. In the U.S., we also have hospice-eligible patients seeking care in the ED and spending their final days in a hospital bed. The article mentioned the following ideas that would improve our care delivery. “Fast Track Hospice Beds to prevent hospital admissions or Hospital Discharge Beds to admit people from Emergency Departments or Urgent Care to prevent lengthy and costly hospital stays.” If healthcare organizations are willing to undertake this approach as an innovation to improve end-of-life care delivery, it would justify even further embedding hospice and palliative care professionals in the ED to ensure hospice-eligible patients receive the high-quality palliative care they deserve to address their symptoms and their families receive the needed support.👇 #palliative #pallitivecare #palliativemedicine #hospice #hospicecare #endoflifecare #hpm #hapc #soinspalliatifs #cuidadospaliativos #palliativezorg #hospicezorg #healthcare #hospitals
