HSJ Online reports that spec comm delegation may be delayed for a further year for the NE and Yorkshire ICBs. No point me being modest here and there is the lovely Geordie phrase of "shy bairns get no toys" so forgive me a moment of self-congratulation. I have been forecasting this for some time based upon the simple fact that the risk of migration is too great for all parties. As recently as late September I said this at a symposium and was contradicted by one person in the audience but then supported in my concerns by many, many, others. We are talking about the most ill and vulnerable people in society, the highest cost services, the highest cost medicines and extremely complex systems of interaction across services. In financial senses Spec Comm is essentially an insurance scheme and a most sensitive one. The proposed changes lacked worked-through detail and the integrity to risk transition. The awareness amongst senior clinicians is negligible and the concern amongst managers is very high. If you play through the practicalities then the absence of assured detail is obvious. As I have said many times, the use of weighted capitation funding is just one aspect that alarms anyone war-gaming through the consequences. Above all, the basic question is how do you ensure the claimed policy commitment of consistency of access to services/medicines when the whole scheme is predicated on ICBs taking on capped budgets that vary per capita? Then you add in the lack of attention to cross-boundary flow practical issues and there is a high risk that this will go wrong, especially in smaller ICBs.
