Fantastic bog by Darren Larsen - Great to see you back to writing and it's a critical time for these conversations. To the points you've raised, I'd like to add three additional emerging trends in public expectation: - Add: The desire for thinking beyond disease management: With the greater access to information and greater self-determination towards health, concepts such as "health span" are increasingly permeating the public. I predict an increasing number of patients will hope that clinicians can better guide patients towards what to do to optimize/prolong health. - Add: Modernize the data with which clinical decisions are made: Right now, "personalized healthcare", while in its infancy, still is outside the evidence and scope of public healthcare. People will however expect over time that modernization of treatment decisions are made (pharmacogenomic prescribing, shift from guideline to individualized preventative health screening, etc) - Add: Care across Canada: In line with your previous point re "On-demand expectations", I believe more Canadians will expect that at some point, just like they can access their Netflix account anywhere in country or get the same experience at an Apple store in any city, they will hope that their health information AND ability to get consistent care is not isolated geographically constrained. A very useful addition to the thinking on evaluation of impact of virtual wards by Melissa Co at The Health Foundation which concludes - “The question of whether virtual wards ‘work’ 'is complex but also part of a bigger picture. An ageing population in England with higher rates of multiple chronic diseases and the shift towards net zero care in the face of climate change mean the NHS needs to radically shift towards more proactive, community-based care. Virtual wards are likely to be a part of this future. “We know our health care system does not work equitably for all,” - Juan Fernando Lopera, compact member and chief diversity, equity, and inclusion officer at Beth Israel Lahey Health. While many Massachusetts residents struggle to access high quality, affordable, and timely health care, systemic inequities and institutional racism exacerbate these issues for many communities of color, said the The Center for Health Information and Analysis (CHIA) in a report issued Wednesday. The results in this report echo the findings of the Blue Cross Blue Shield of Massachusetts Foundation report which detailed that our state spends $1.5 billion dollars on avoidable healthcare costs as a result of racial and ethnic inequity. In response to the deep-rooted, unacceptable disparities in health and healthcare, we seek to drive change through the bill, An Act to Advance Health Equity, which would mandate reporting of health equity data and enact a number of protective measures to improve health outcomes for those facing systemic barriers to equitable health outcomes, as well as all our neighbors across Massachusetts. We thank CHIA for taking a detailed look at the racial and ethnic differences in hospital utilizations across settings and advancing the important conversation about the reasons behind the data and trends.
