ICYMI: Aledade, Inc. is the largest network of independent primary care organizations in the country, helping primary care practices, Community Health Centers (CHCs), Federally Qualified Health Centers (FQHCs), and health clinics deliver better care and thrive under value-based care. The utilization of ADTs supports transitional care management (TCM) to improve care coordination, prevent avoidable readmissions, and reduce costs. Approximately 1 in 7 TCM follow-ups prevent hospitalization, which saves approximately $15,000 in medical spending per admit. In 2022, ADTs provided by MX triggered approximately 2,000 TCM visit follow-ups and around 2,400 ED follow-ups, resulting in approximately $4.2 million dollars in savings while improving the quality of life and clinical outcomes for patients. We recently collaborated with Transforming Care Partners to take stock of palliative care in California - where we've made progress and where there are still opportunities in the years ahead. Over 30 leaders across health plans, providers, community organizations, government, and associations shared insights based on their deep experience. This summary reflects the analysis and CHCF’s expertise on two fronts - the catalysts driving recent gains as well as priorities to accelerate future growth. Palliative care in California is at an inflection point, transitioning from startup mode but not yet an integrated standard of care. By reviewing what has worked and discussing stakeholder actions to maintain momentum, we aim to further strategic conversations on sustaining progress for equitable access to high-quality, person-centered palliative care across California over the next 15 years.
