This gives an emerging doctor the opportunity to: 🩺 undertake specialised training at South West Cleft Services Bristol, in the complete range of cleft lip and palate surgery 🔬 facilitate important research in conjunction with The Cleft Collective (the world's largest Cleft Lip and Palate research programme) Thanks to the generosity of our donors and supporters, Dr Nitisha Narayan completed her Bristol Cleft Fellowship last year and presents the findings that took place for her study 'Investigating if Cleft Lip repair in older children is associated with higher complication rates". For this study, a retrospective review of patients undergoing primary Cleft lip Repair was performed for patients between 2020-2022 Aug (during the COVID19 pandemic). To compare the outcomes, they collected data from patients operated between 2018 and 2019. During the pandemic 100% of the patients undergoing primary lip repair were over the age of 6 months compared to 16% (8/50) in the pre-pandemic group. In the pandemic group, the parents of 12 children (21%) reported an incident causing trauma to the lip repair, which was statistically significant. We conclude from this study that Cleft Lip surgery performed in older children has the potential to cause significant and unwanted complications. We are proud to present our new paper called: ‘’Limited evidence in support of bone marrow aspirate concentrate as an additive to the bone marrow stimulation for osteochondral lesions of the talus: a systematic review and meta-analysis’’ in the KSSTA Journal. This project started as my bachelor thesis at the department of orthopaedics and sports medicine of the Amsterdam UMC, and continued to become my first published article. Link: https://lnkd.in/dvp4ZZmb Key findings: - Currently there is insufficient evidence to support a positive effect on clinical outcomes of Bone Marrow Aspirate Concentrate as an additive to surgical treatment of osteochondral lesions of the talus. - An interesting significant difference in clinical outcomes was found in the subgroup comparing Bone Marrow Stimulation with Bone Marrow Stimulation + Bone Marrow Aspirate Concentrate. However, the level of evidence was weak. - Based on the safety reports and initial results, sufficiently powered, patient- and researcher-blinded, prospective randomised controlled trials are justified and recommended. Excited to share our latest research publication, "Impact of Hospital Affiliation With a Flagship Hospital System on Surgical Outcomes," now available in Annals of Surgery. Our study dives into the nuanced effects of hospital system affiliations on patient care, specifically looking at surgical outcomes within flagship hospitals compared to their affiliates and matched controls. Key Findings: -Patients at flagship system hospitals exhibit significantly lower mortality rates post-surgery than those treated at non-affiliated hospitals. -This outcome advantage is particularly pronounced in flagship hospitals themselves, highlighting the importance of hospital selection for patients, especially those with multimorbidity. -Surprisingly, affiliate hospitals within these flagship systems did not demonstrate the same level of outcome superiority, suggesting that the benefits of flagship affiliation may not uniformly extend across all networked facilities. This work underscores the critical need for patients and policymakers to consider the varying levels of care within hospital networks. Join Medics4RareDiseases and FOP Friends for a unique evening reception, at the prestigious Hunterian Museum, at the Royal College of Surgeons of England on 13th February 2024. 'First, Do No Harm: Expert Reflections on a Rare Bone Disease' is an opportunity to learn about how patients and families contribute to a greater understanding of rare disease through the story on one rare bone condition, Fibrodysplasia Ossificans Progressiva. Clinicians, patients and advocates will share stories about the lived experience of people with FOP from the 18th century to the present day. Attendees will also be able to visit the Hunterian Museum and the skeleton of a man who lived with this condition in the 1700s. FOP is an example of a rare genetic condition that has been familiar to medicine for hundreds of years, but is not familiar to the average healthcare professional. It is characterised by the abnormal development of bone in skeletal muscles and connective tissues, progressively locking joints in place and limiting movement. However without healthcare professional awareness of FOP the basic tenet of medicine, ‘First, do no harm’, is easily missed. When living with FOP any trauma, including surgery, can trigger a flare response resulting in abnormal bone growth in the affected area. You will hear from clinicians and advocates who have been pivotal in working towards treatment and improving the lives of those living with this rare condition. The American Physical Therapy Association (APTA) #CSM meeting coming up next month reminded me of a publication from last year led by Matthew Smith and Jane Borgehammar, with some compelling results for consideration. Scientific conferences are a great opportunity to share innovation and cutting edge research with the greater clinical community. BUT....these results are often not fully vetted; nor have they gone through peer review. In their review they identified all research abstracts presented at the APTA CSM orthopaedic and sports sections from 2012-2017 (fortunately, these abstracts are published by the Journal of Orthopaedic & Sports Physical Therapy (JOSPT) each year in a supplement). They then looked to see how many of these went on to be published in a peer-reviewed journal. This is an important step to validate those results, and make the information available and useful for implementing policy and clinic-level change. Often researchers are finishing up a preliminary analysis just in time to meet abstract submission deadlines. Later when completing their manuscripts, or even during the peer review process, errors may be uncovered and the results and conclusions in a publication can be quite different than what was presented at the conference. After excluding case studies, there were 2436 abstracts presented during these years. During the following 5 years, only 47% went on to publication (60.4% in 2012 to 32.8% in 2017). What caught my attention the most was that only 24.2% of systematic review abstracts and only 61.9% of randomized clinical trials abstracts went on to be published. These are the top of the evidence pyramid, and the studies most likely to make substantial impact. Clinical trials are also the most expensive to conduct (cost and manpower), so it seems a waste to not have the full results of the study available to the general public. All of this is not to say that sharing research at conferences has no value. Of course it does. It's a practice that should continue. As a consumer, just keep the results from this study in mind as you engage with and interpret results from abstract presentations you attend next month (btw, this has been studied in multiple other medical disciplines and they are not any better). Hello colleagues, I want to let you know about a unique training course opportunity on superficial venous disease on the Caribbean Island of the Dominican Republic.   Patients on this island have extensive superficial vein disease requiring diagnosis and treatment. Since the beginning of this course there have been over 400 participants that have actively participated in this training course. Course overview:  We offer an intensive 2.5 days of hands-on training, that said, participants will utilize their newly acquired skills to perform venous procedures under our direct supervision. This course is designed and limited to 2-3 participants at a time.  We provide one-on-one individual instruction and this ensures participants perform a high number of individual cases. Participants will perform phlebectomy, ultrasound guided enhanced injectable foam sclero therapy, and visual sclero therapy. In addition, prior to the procedure, participants will perform the venous duplex mapping exam.  Radio-frequency ablation training can be arranged. Didactic lectures and cases are presented at breakfast, lunch & dinner.  All aspects of venous diagnosis, treatment and management are reviewed.  Participant’s will stay at a state-of-the-art luxury villa and meals are prepared by the executive chef.
