It's high time we stopped referring to the proper administration of therapeutic oxygen as a "MIRACLE." The benefits are well-established and extensively documented. Calling it a miracle undermines its credibility and hinders its growth and adoption. The FDA has approved insurance coverage for 14 conditions in the U.S., and hyperbaric oxygen therapy has been proven effective for countless other conditions as well. We’re not living in the Middle Ages, where science was a miracle. Let's embrace the therapeutic use of oxygen to enhance the quality of life for our citizens, beyond just the Medicare-approved $2000 per treatment. Let's also clear up a common misconception: medical-grade HBOT is not the same as mild hyperbaric treatments. Physicians and chiropractors practicing mild hyperbaric therapy should face serious scrutiny—perhaps even having their licenses reconsidered. The public deserves accurate information and effective treatments, not diluted versions that don't meet medical standards. Why is it that companies from Israel are setting up shop here, particularly in The Villages, and billing our taxpayers $43 million over the past four years while we’re served pressurized air useful at treating acute altitude sickness? It's time we take a breath, quite literally, and start utilizing this proven therapy more broadly and efficiently here at home. Published evidence suggests increasing your fibre intake by 10g a day  (two table spoon of chia seeds, or a pear and some lentils or a banana) can lower your bowel cancer by 10% a tiny change with an incredible payout, and six to eight hour of sleep.All you need to know is: aim for a consistent sleep and wake up time, and you know you’v had a good night’s rest if you wake up feeling refreshed. How to explain PPPD - How I describe symptoms to patients ✅ I start by standing on the floor in front of the patient ✅ From this position, I start to challenge my balance. I start wobbling a bit, I put my feet close together and I stand on one leg ✅ I then ask what the patient thinks would happen to my balance if I did the same thing but stood on a table ✅ The patient usually responds “...you will probably experience that your balance has deteriorated” ✅ But what have I changed from standing on the floor? My vestibular system does not know that I am standing on a table and the surface is the same for my feet. ✅ I have assessed the situation differently and in this case my brain "alarms" when it sees a potential threat (the height) ✅ This means that sensory information from my feet and my vestibular system from my ear can work perfectly but I can still experience symptoms of unsteadiness ✅ What I experience when I stand on the table are similar symptoms that you can experience on the ground. This means your brain sends "alarm signals" in situations that are not a real threat. You can say that the brain is temporarily "stuck" in a high-risk behavior. ✅ It is also in this way that the patient can explain his symptoms when he gets home so that relatives can understand the symptoms better. "The discomfort that you can experience at a high altitude is what I can feel when I stand on the ground” ✅ "The stand-on-a-table-explanation” can facilitate understanding of the symptoms for some patients suffering from PPPD. Let's talk habits! Behavioural changes don't happen overnight, however, a decision to change old habits, might! Simply planting a small seed could lead to a 180° change in someone's lifestyle. At Perioperative Medicine University Hospital Southampton, we take pride in our holistic approach to patient care which is second to none. During their journey with us, regardless of the nature of the patient's appointment (CPET, Perioperative Anaemia, Shared Decision Making/High Risk Clinic etc), our team uses Early Screening tools to assess overall fitness to enable healthy conversations. Recently, I have been supporting our IV Iron clinic, and, in two days, with just a simple conversation to explore lifestyle choices, I have positively influenced: -A patient to cut down alcohol intake before surgery (reported drinking 1 bottle of wine/day since retirement, over 30 years ago). -A patient to engage in smoking cessation programme. (reports smoking all his life, 20 cigarettes/day) -A patient who is uncertain about proceeding with surgery or going down the route of radiotherapy, to increase their activity levels to prepare for either of the options as exercise has proven effective and positive to cope with surgery but also with chemo/radiotherapy, decreasing the chances of deconditioning. (All of these interventions will be followed up and the patients appropriately supported through this process). Holistic approach includes education and health promotion. Behavioural change takes someone to plant the seed, to question the habit. Most of the time, the patients are open to listening, open to suggestions and most certainly, open to be educated on the importance of their habits and its impact in their health. This is the 2nd most popular topic I am having discussions about with health professionals right now. (The first is lifestyle medicine to improve outcomes and contain costs around GLP-1 meds). Lifestyle medicine and health coaching has always, and still does, struggle in the world of reimbursement and insurance. Value based care is one way it can happen. We will see Value based care plans like Wellvana continue to grow as the baby boomers continue to retire - it’s the payers way of lowering cost. Placing it back on the provider or end user be responsible. I hope it works. It can. 1. Group programs should be made available to individuals enrolled in these contracts. 2. If a patient goes to the ER one time or is on more than 2 medications for any chronic illnesses in any given year, they should be assigned a health coach. This study shows that during the Early Intensive Neurorehabilitation program, a large percentage of people with PDOC regain at least a minimal conscious state or even consciousness. These outcomes and the frequent medical complications in this group suggest that intensive specialized care should be offered to all people with PDOC. The findings of this study may improve the medical care/management of people with PDOC and might help health professionals to inform the families of people with PDOC about the short-term prognosis of PDOC. Future research into long-term outcomes, quality of life and cost-effectiveness is necessary for further improvement of care/rehabilitation following PDOC and is on the way, Danielle Driessen Cecile U.
