Antibody-Drug Conjugates (ADCs): A Journey from Concept to Clinical Reality AADC has come a long way in research, however, very much picked-up in a last two decades after the first FDA approval of Mylotarg in 2000. ADCs represent a remarkable convergence of immunology, chemistry, and oncology. Their journey from theoretical concept to clinical reality has been both fascinating and transformative. Here is what you need to know about #ADC #scienceinshort Inception and Early Exploration (1960s-1980s): The idea of using antibodies as targeted drug delivery vehicles emerged in the 1960s. However, practical implementation remained elusive. In the 1980s, researchers began to explore ADCs more systematically. The challenge was to find the right balance: an antibody that could recognize tumor-specific antigens and a payload that could selectively kill cancer cells. Mylotarg: The First FDA-Approved ADC (2000):Mylotarg (gemtuzumab ozogamicin) achieved a significant milestone by becoming the first FDA-approved ADC. It targeted CD33, an antigen expressed on acute myeloid leukemia (AML) cells. Despite initial enthusiasm, Mylotarg faced safety concerns related to liver toxicity, leading to its temporary withdrawal from the market. Challenges and Innovations: ADCs encountered hurdles such as instability, off-target effects, and premature drug release. Researchers focused on improving linker chemistry, optimizing drug payloads, and enhancing antibody engineering. Recent Advancements (Last Two Decades): Conditionally Active Biologics: Novel ADC formats allow activation only within specific conditions (e.g., tumor microenvironment), minimizing side effects. Potent Payloads: Researchers explore highly potent cytotoxins, aiming for better tumor cell killing. Stable Linkers: Advances in linker design reduce premature drug release and enhance overall efficacy. Site-Specific Conjugation: Precise attachment of drugs to antibodies improves homogeneity and consistency. Clinical Pipeline and Beyond: ADCs continue to evolve. Intracellular Delivery: Nanoparticles can bypass drug efflux pumps (a common mechanism of resistance) by delivering drugs directly inside cancer cells. 4. Imaging and Diagnosis Theranostics: Some nanoparticles are designed to have both therapeutic and diagnostic capabilities. These theranostic nanoparticles can help in imaging tumors, delivering therapy, and monitoring the response to treatment in real-time. Enhanced Imaging: Nanoparticles can improve the contrast in imaging techniques like MRI, PET, and CT scans, allowing for better visualization of tumors. 5. Types of Nanoparticles in Cancer Therapy Several types of nanoparticles are used in cancer treatment, each with specific advantages: Liposomes: These are spherical vesicles with a lipid bilayer, used for encapsulating drugs and protecting them from degradation. Polymeric Nanoparticles: Made from biodegradable polymers, these nanoparticles can provide sustained release of drugs. Gold Nanoparticles: These are used for both drug delivery and as agents in photothermal therapy, where they convert light into heat to kill cancer cells. Magnetic Nanoparticles: These can be directed to tumor sites using external magnetic fields and used for hyperthermia treatment, where localized heating destroys cancer cells.

Abstract: Antibody-drug conjugates (ADCs) represent an important class of cancer therapies that have revolutionized the treatment paradigm of solid tumors. To date, many ongoing studies of ADC combinations with a variety of anticancer drugs, encompassing chemotherapy, molecularly targeted agents, and immunotherapy, are being rigorously conducted in both preclinical studies and clinical trial settings. Nevertheless, combination therapy does not always guarantee a synergistic or additive effect and may entail overlapping toxicity risks. Therefore, understanding the current status and underlying mechanisms of ADC combination therapy is urgently required. This comprehensive review analyzes existing evidence concerning the additive or synergistic effect of ADCs with other classes of oncology medicines. Here, we discuss the biological mechanisms of different ADC combination therapy strategies, provide prominent examples, and assess their benefits and challenges.
