🐮Calf raises with big toe pain🦶 For those with 1st toe pain or limited range of movement, standard calf raises can be uncomfortable. So what other options do we have to offload this joint? 1️⃣ Perform in shoes: While we usually prefer to perform calf raises barefoot, doing them in shoes, especially those with a forefoot rocker, can ease load at the 1st toe joint. 2️⃣ Arrange books, weight plates or pieces of wood to give room for the 1st toe whilst still providing support underneath the toe 3️⃣ Stand on the edge of a step, book or weight plate so that the entire 1st ray is off the edge. This option is more challenging for balance. Achilles paratenon irritation: what is it & how to fix it!🤕 The paratenon is a sheath that covers the Achilles’ tendon which can get irritated & cause tendon pain. Its’ function is to protect the Achilles tendon & reduce the friction between the tendon & adjacent structures. What are the telltale signs of paratenon irritation? 🔑Diffuse pain up & down the tendon vs more specific pain in tendinopathy 🔑Pain is worse on slower movements i.e. calf raise rather than quicker movements such as hopping (tendinopathy will be worse with hopping) 🔑Crepitus (noise) that can be felt or heard (with the naked ear or stethoscope). 🔑Puffiness & swelling around the tendon The solution? 🎯Applying a mix of topical anti-inflammatory & hirudoid cream twice per day for 7-10 days, wrapping in cling wrap overnight (only apply if directed by a health professional) 🎯Appropriate loading progressions: those suffering paratenon irritation may better tolerate isometrics or more dynamic exercises such as hopping (rather than slow calf raises), as there tends to be LESS sliding of the tendon inside the sheath with these movements. ❗️Don't ignore the kinetic chain in calf & Achilles injuries!🤕 It's not uncommon for those with recurrent calf & Achilles injuries to display strength deficits throughout the entire limb. This was supported by a 2022 study by Sancho that showed weakness in the quads, hamstrings & glutes in those with Achilles tendinopathy. 📈Loading is highest on the calf & Achilles during the midstance of the gait cycle. If we look to see what other muscles are active during this phase we see the quads, hip extensors (hamstrings & glutes) & hip abductors play a role in supporting the body. 🏃♀️In theory, if these muscles are not providing adequate support, then greater stress could be placed on the calf & Achilles. According to the BMJ: “Neck pain is a highly prevalent condition that leads to considerable pain, disability, and economic cost. It not only constitutes a major personal burden but also affects families and the health system and economic structure.” In referrals I receive from fellow Physiotherapists, GPs and Consultants, Neck pain comes second to Back Pain referrals but causes the same misery, fear and worry. Having severe neck pain in my early thirties lead me to change direction in my career and lead me to Pilates . I started to really understand and know my body, its strengths and limitations. Doing Pilates removed the fear of my neck pain as it released its grip, alleviated the fear and has enabled me to prevent and manage flare ups and this is when I started to teach Pilates , wanting others to manage their Neck Pain.
