#CBCLIPS Episode 48 Case Based Clinical Learning In Pediatric Surgery A 9-month old girl with Trisomy 21 presents with acute non-bilious emesis, worsening over 5 days. She has a history of intermittent vomiting and poor weight gain since birth, diagnosed and treated as gastro-esophageal reflux disease. Exam shows a deyhdrated, but non-toxic baby weighing 5.4 kg (15th percentile). Her abdomen is soft and non-tender. Laboratory tests show a severe hypokalemic hypochloremic metabolic alkalosis and hemoconcentration. A films is shown below. How would you proceed with this patient? As we recognize Head and Neck Cancer Awareness Month, a member of MOPSS shares their journey with HNC. After receiving biopsy results, they were recommended to undergo a long and grueling multimodality treatment. However, before surgery, they needed dental work, which they were able to pay for out of pocket. They analysed 12,942 TKA patients, defining stiffness as patients requiring a manipulation under anaesthesia (MUA). Patients with stiffness incurred costs 1.5 to 7.5 times higher than non-stiff patients in the 2 years following TKA. I am proud to share that as a member of the APSA Outcomes and Evidence-based Practice Committee, we have published a significant systematic review and evidence-based guideline in the Journal of Pediatric Surgery! Title: Classification and Surgical Management of Anorectal Malformations: A Systematic Review and Evidence-based Guideline from the APSA Outcomes and Evidence-based Practice Committee 🔍 Highlights: • Treatment of neonates with anorectal malformations (ARMs) is challenging due to variability in definitions, surgical approaches, and outcomes. • Our review summarizes existing evidence, identifies treatment controversies, and provides guidelines for perioperative care. • We reviewed 10,843 publications, with 90 included in our final recommendations. • Both loop and divided colostomies are viable options, though loop colostomies have higher prolapse rates. • No significant differences in outcomes between early and late repair groups. • Clear and uniform definitions are essential for future comparisons and recommendations. 📈 Conclusion: Evidence-based best practices for ARMs are still lacking in many areas. Multi-institutional registries and further prospective studies are crucial to improve care and develop consensus guidelines for this complex patient population.
