Please read the following article I wrote regarding a CMS proposal that will significantly impact the labor market for hospitals and nursing homes.

"2024 Long Term Care Nurse Staffing Study (LTCNSS) During the spring of 2024, the Texas Center for Nursing Workforce Studies (TCNWS) at the Department of State Health Services is asking all directors of nursing facilities in Texas to complete a short survey. The information collected will be used to: Assess the size and effects of the nursing shortage in Texas' long-term care nursing facilities. Forecast the number of nurses needed in Texas. Serve as a resource for the Texas Center for Nursing Workforce Studies’ Advisory Committee, the Governor's Office, and legislators in establishing legislative priorities and making policy decisions. Please be aware that your survey responses will be strictly confidential. All findings will be reported in aggregate form only. Individual agency data will not be identified. Emergency Department Boarders 🛌 These are the patient's who are waiting. Waiting for an admission doctor. Waiting on a room in the hospital. Waiting to be transferred to rehab. Waiting for a board and care to accept them. Waiting for a psych bed to open up. Just waiting... waiting... ...and blocking rooms for Emergency Department patients. It's an epidemic nationwide, and a plethora of factors affect the situation: staff shortages, budget cuts, infrastructure logistics, EMS overload, etc etc. While definitely frustrating, the ER nurse must remember the patient above all. Were their daily medications ordered, or are we just waiting for them to get symptomatic before we request the meds? Did they get a shower or even wet washcloths for a sponge bath? Are they being turned and getting off those bony prominences? Are we treating them like humans? Emergency Department nurses tend to gravitate towards the adrenaline, and boarders just aren't exciting. But they ARE still patients requiring regular, good nursing care. Failure to perform basic nursing care simply because they've been stabilized is unacceptable, and ER nurses are capable of being the boarding patient's nurse, whether that is ICU, med/surg, telemetry, or wound care. 'Those people aren’t smart enough to be trusted...' Last year, at a gathering of fellow nurses, someone directed that statement at me. We were convened to learn how to educate and empower Ohio nurses in shaping state policy and legislation. Amid discussions on tackling the staffing crisis, I delved into the role and potential of Medication Aides (MA-Cs) in Ohio's acute care facilities. A bit of context: In 2006, non-nurses were authorized to administer certain medications in nursing homes, assisted living facilities, and residential care facilities. What began as a pilot program soon expanded statewide by 2009. Yet, since then, progress in reevaluating and expanding the scope of medication aides has stagnated. All that said, the current staffing shortage is much more nuanced than numbers. Its complexities cannot be solved with such a narrow solution. Still, I proposed exploring tangible, quick-turnaround solutions to gauge their impact on acute care. After presenting this idea, I was pulled aside and told “It’s a great idea but won’t work. Those people aren’t smart enough to be trusted with our patients.” Do we assess patients' value based on age or their care setting? This incident showed me we still have a long ways to go in nursing. It shed light on a prevailing mindset for some nurses—a mindset entrenched in outdated paradigms, viewing the world through cracked lenses and taped bridges, desperately clinging to keep them together. The major shortage of nurses in hospitals across New York State is a major concern. We’ve made it our mission to uncover — and address — the reasons why. We’ve just launched our brand-new report with the Center for Health Workforce Studies on New York’s nursing shortage, exposing the major factors contributing to the shortage and the best strategies to address them. 🩺 Celebrating the Vital Roles of All Nursing Home Staff 🩺 A recent study published in JAMDA highlights the incredible impact that different nursing home staffing types have on resident care. This research is crucial as new staffing regulations are considered and care quality is optimized. Key insights from the study: Registered Nurses (RNs): Their presence is associated with fewer hospitalizations and better care for pressure sores. RNs play a critical role in communicating behavioral concerns and initiating appropriate treatments. Certified Nursing Assistants (CNAs): Increased CNA hours lead to decreased antipsychotic drug use and improvements in daily living activities. CNAs provide essential daily care and develop insights into person-centered care routines that significantly enhance resident well-being. Licensed Practical Nurses (LPNs): While not highlighted as frequently as RNs and CNAs, LPNs are crucial in supporting both groups and ensuring the smooth delivery of care. Each category of nursing staff brings unique and valuable contributions to resident care, highlighting the need for a balanced and well-supported team. Hiring decisions should consider the specific strengths and roles of RNs, CNAs, and LPNs to meet individual resident needs effectively.
