Today this amazing team won MultiCare’s CEO & President's Clinical Patient Outcomes & Quality Award for our Puget Sound Region! Their work this year on creating a clinical pathway for Sedation & Delirium prevention at MultiCare’s Tacoma General’s Neurotrauma Intensive Care Unit (NTICU) has now been adopted system-wide. This clinical pathway has had a huge impact on the amount of time patients require mechanical ventilation. Less ventilation time means less sedation with fewer of the harmful side effects like delirium. Part of the larger “QA-44” program I’ve been directing since 2022, this amazing interdisciplinary team has been steadily been improving our performance in the four domains of healthcare’s Quadruple Aim. We started with a mandate to improve patient and clinician wellbeing through a focus on better outcomes for our patients knowing that doing the right thing for our patients and staff would deliver dividends for the system. On every initiative the team listens to clinical champions and identifies where variance and inefficiency are impacting quality. They then work closely with local leaders implementing evidence based clinical initiatives using rapid PDSA cycles rooted in improvement science. So why do they deserve an award? Doing the right thing has really paid off, faster recovery times and shorter patient stays has meant lower bills and improved ICU capacity for these critically ill patients. Nearly $8M in total benefit and savings have been identified so far this year. Wow! Even more importantly, they are preserving the health and well-being of the patient. The NTICU team is a role model for living MultiCare core values. The NTICU leadership teams foster team development, growth and wellbeing while improving patient care, and making a difference in the lives of their patients. Hospitals in the US are restructuring, and the span of control for those in executive leadership positions is expanding. The workforce may wait longer for a sighting from their hospital President when he leads four major medical centers in Boston. An Interim colleague will now lead nine hospitals in the community within a 90-mile radius; with turnover rates rising, some staff may never meet him. I imagine the leadership structure reporting to each medical center President has changed, with everyone assuming more leadership responsibilities. The trickle-down effect may result in a wider gap between the Administration and the workforce - >responsibilities = >meetings. With clinicians and leadership doing more with less, what will 2024 bring for hospitals and patients? "Our leadership updates will allow us to better deliver on our strategic goals, including providing extraordinary care to all our patients, shaping the future of healthcare through science and innovation, and providing 70% of care in the community by 2030," Kevin Tabb, MD, president and CEO of BILH, said in the release. I got a chance to attend 2 awesome conferences in the last months, #fmx2023 and #NERUCA2023 hosted by American Academy of Family Physicians and NERUCA - North East Regional Urgent Care Association, respectively. It's been a month of learning, making connections, and having fun. My main focus was to find the biggest challenges that primary care and urgent care practices are facing and see how #POCUS can become part of the solution. Here are 2 major TAKEAWAYS to be expanded upon in the future posts: 1. Urgent Care patient volumes and reimbursements are down. It's becoming increasingly difficult to negotiate reasonable contracts with private payors. So why not offer POCUS to improve care, keep imaging in-house (and thus the revenue in fee-for-service contracts), and attract new patients while building patient loyalty? 2. Value-based care is becoming the norm among primary care practices. This means patient-centric care that saves costs. Do more effective and efficient medicine, stop sending patient to the ED if you can avoid it, etc. Well, you guessed it, POCUS is a perfect solution for certain presentations. Do a compression ultrasound study to r/o a DVT (blood clot) in the office and keep them out of the ED. Do a shoulder ultrasound in the office and don't order a more expensive MRI. Give patients answers at the bedside, start them on appropriate treatment, and let them get better at home, not the hospital. Please comment below on what's your experience has been with POCUS and reimbursement in general. What are you strategies to fighting private payors to agree on reasonable rates?
