Good article about the growing pains of Value-Based Care. If you're looking for immediate outcomes and savings, adopting VBC isn't it. But isn't fixing the system in the long run worth it? The author recommends 4 focus areas: 1) Prioritize the training of clinicians, 2) Use incentives effectively and early, 3) Build a cooperative and collaborative culture, and 4) Help clinicians utilize data efficiently. "The transition towards a value-based model of healthcare is challenging and labor-intensive. Not only does it involve changing workflows, procedures, and payment models–it often entails a ‘hybrid’ period where organizations start offering a value-based model while continuing to operate within the conventional fee-for-service model. Great overview of key considerations associated with value-based alliances between payers and provider groups. While the article highlights independent providers, the same principles hold true for IDNs and employed physicians as they prepare for the future. Awesome to see how payers and provider groups are aligning to advance value-based care in the US.

Fee-for-service models create a major dilemma in healthcare. As Mohamed Diab explains, "...patients want more access to coordinated, convenient, personalized, and affordable care, but physicians have limited time and resources," and "For physicians, the promise of value-based care is the ability to spend more time focusing on the whole patient." Large systems will likely not be able to effectively provide for each patient an ongoing individual care plan. What is the best way to judge a #valuebasedcare model explored by #CMS? If the results don't line up with initial expectations or don't hit all of the targets is it a failure? Could it just be that the metrics for success should be viewed differently or that different lessons should be learned? Those are some the questions that should be considered following an analysis of #Medicare data available from the CPC+ model. While #primarycare spending increased in both tracks, #patient use of the emergency department and #hospital stays decreased. Is that a problem or does it show that #proactivecare may cost a little more upfront, but reduces higher spend from more intensive, reactive services? Here's the gyst of the article: Medicare's Chronic Care Management (CCM) program claims to reward doctors for managing patients' health outside of office visits. However, uptake has been low; in 2019, only 4% of eligible enrollees participated. While the program offers additional income for doctors, challenges like administrative burden, documentation requirements, and patient opt-in hinder its success. Some doctors find the program cumbersome, requiring integration into electronic health records and hiring additional staff. Outsourcing to private companies has been a solution for some healthcare systems, making care management more feasible - yet still with mixed reviews. Despite that, the CCM program has potential benefits, including reduced hospitalizations and improved patient monitoring.
