We think that "primary care" lost it's meaning as a foundational "relationship" and plain old "my doctor and their team" when healthcare became so specialized that interaction was mainly reduced to break-fix, annual checkups, and broken care-continuity due to the tangled specialist referral process that is driven by payers restricting access. Remote and hybrid telehealth as an adjunct to, or even perhaps the primary engagement modality will only win the hearts and minds of consumers if there is a return to "trust" and "continuity of care" and "continuity of relationship" beyond the break-fix and the perfunctory annual "encounter." Really appreciate your attention to this kind of whole-person care and easy access to care and relationships with the care team, absent barriers and hoops and access friction. #goodwork

Congratulations on this merger. Thought the focus of the discussion was spot on, emphasizing that members trust their primary care teams, which needs to be fundamental to everything being offered. Ashok Subramanian appreciated your reference to the “Grand Bargain” at Centivo, where members agree to engage in the primary care experience and, in exchange, will gain access to the most affordable health plan and the best overall experience. One of the draws of subscription-based direct primary care and concierge medicine is the relationship with and access to one's own personal physician with whom one has a relationship (as contrasted against a team of midlevel practitioners overseen by a physician one never actually sees). As you scale how do you deal with the very real financial and operational pressures that so often get in the way of maintaining that important direct personal relationship between patient and physician?
