So, I just finished up a little book that a friend recommended... Oddly, it may have answers for the backlogs and difficulties we are already experiencing in the medical system and why the current "solution" of just getting more doctors might not work very well. The book is called "The Bottleneck Rules". It references "The Goal", a 1984 book about production line management... And why adding faster, better machines may make NO difference to the overall production speed. Depending on what is really happening - adding more doctors MAY not influence the backlogs in the medical system. For instance, emergency departments commonly fill up with "too many patients," yet adding more doctors won't fix the issue if the bottleneck is in the availability of beds in the hospital. Finding ways to clear beds more quickly or limit the number of people entering the hospital will fix it. Now, there will always be a bottleneck in the system unless all steps run at exactly the same speed and the raw materials in the system never run out. The real art of the book comes in later when strategically choosing the best bottleneck. Ignoring possible risks of being spread too thinly, Clarke talks about the French surgical system, which has one surgeon running two theatres to ensure the surgeon is working at maximum capacity - not waiting for the patient and theatre to be ready... Now, there may be additional risks of not allowing enough rest or what to do if there is an emergency in one theatre - not sure how they cope with that! Of course, we did not get any bottleneck production training in med school, yet this may be a critical skill in resolving the coming (present?) demands on the Australian health system. So, being able to recognise the bottleneck and make clear decisions around whether it should be the bottleneck and, if not, what to do about it may be a valuable skill set for all healthcare practitioners in the coming years!

When one single patient's life can be saved, it's a fantastic day. Recently, the AHVAP Certification Center received an email from a student in our Medical Device Safety micro-credential program: "I am so thankful that I was fortunate enough to benefit from the recent AHVAP Certification Center Medical Device Safety. Recently, our health system was going to implement a new medical device in our ICUs that was requested by our clinical teams. Because we implemented the medical device safety checklist from the course, we were able to perform some critical research prior to implementation and determine that there were some MAUDE events that would have potentially negatively impacted our patient population. Our ICU physicians were very thankful that we collaborated with them to prevent patient harm as part of our value analysis process and were able to be proactive. As a result of this approach, we were able to quickly pivot to a different device that still met the clinical need and did not have the same high risk safety profile in the literature.
