Type 2 diabetes is a chronic illness that affects millions of people and dramatically increases their risk of cardiovascular disease. The rising prevalence of type 2 diabetes has led to increased research on lifestyle and dietary interventions, such as very-low-carbohydrate ketogenic diets (VLCKDs), to reduce cardiovascular disease risk. A recent systematic review and meta-analysis of 29 clinical trials analyzed the impact of a ketogenic diet on cardiovascular risk factors in people with type 2 diabetes. The trials included both ad libitum and calorie-restricted interventions. Adherence to VLCKDs led to the following results: → Significant reductions in fasting blood glucose, HbA1C, HOMA-IR index, and insulin concentration overall, although results varied among studies → Significant reductions in triglyceride levels across all studies → Significant reductions in HDL levels overall, although results varied among studies → Significant reductions in systolic blood pressure across all studies and diastolic blood pressure in some studies → Non-significant increases in LDL cholesterol levels, although results varied among studies This review found that adherence to a very-low-carbohydrate ketogenic diet improves glycemic control and reduces cardiovascular risk factors in people with type 2 diabetes. Anybody interested in keto diets? I certainly use the principles of a ketogenic diet with certain patients who have very poor metabolic health, insulin resistance, obesity and metabolic syndrome. This study suggests that a ketogenic diet in mice increases markers of cellular senescence - sometimes called “zombie cells” in mainstream media! These cells are one of the hallmarks of ageing and is thought to contribute to many of the diseases of ageing. The study shows that this occurs irrespective of the age of the animal, or whether the keto diet was mostly saturated (bad) or unsaturated (healthy) fats. Interestingly however the markers of senescent cells only rose after a continuous keto diet, and that an intermittent keto diet prevented this problem from occurring altogether. If this is in fact true in humans too, this highlights an important point about biology and medicine. It’s that there are rarely perfect solutions, but rather tradeoffs when it comes to us intervening. Particularly when the intervention is particularly unusual or extreme, for example the ketogenic diet could be considered to be a little…extreme. Near zero carbs?? Most people are terrified by the thought! We also can be misled as to what is the best thing to do if we only focus on one biological pathway or marker of health/disease/ageing. The ketogenic diet is as I said, a tool. We have to decide when to use this tool in the right context for the right patient. For example, my metabolically unhealthy patient who may be, say 50 years old, is much closer to facing the consequences of their metabolic health than they are the effects of senescent cells. The overall net effect of the ketogenic diet for such an individual is likely net positive. Certainly when used as a tool in the short term. Understanding the individual balance of risk for a given person and personalised medicine is the way forward. Thrilled to announce that our research, "Dietary anthocyanins mitigate high-fat diet-induced hippocampal inflammation in mice," has been accepted for publication in the Journal of Nutrition! This study delves into the potential of anthocyanins to counteract the negative effects of a high-fat diet on brain health.  High-fat diets have been shown to increase the permeability of the intestine, allowing for toxins to enter the bloodstream. This process has been shown to negatively impact the brain. In particular, it has the capacity to impair the function of the hippocampus, the brain region necessary for learning and memory. Our findings reveal that anthocyanins have the capacity to reduce hippocampal microgliosis and inflammation, prevent HPA axis dysregulation, and increase hippocampal BDNF levels. Fructose is a major dietary culprit in the development of cardiovascular disease 🩺 The Global Challenge of Atherosclerosis: Once primarily a concern for Western countries, atherosclerosis now affects diverse ethnic backgrounds and age groups worldwide. Despite significant advancements in controlling traditional risk factors, such as hypercholesterolemia, cardiovascular mortalities continue to rise. This trend prompts a critical reassessment of our dietary strategies in combating this pervasive disease. 📉 The Cholesterol Paradox: For the past 50 years, we've successfully reduced dietary cholesterol intake and lowered plasma cholesterol levels through effective, low-cost therapies. However, this hasn't translated into a corresponding decrease in cardiovascular deaths. This paradox suggests that we may need to look beyond cholesterol as the sole dietary culprit. 🍬 Sugar's Hidden Impact: In the 1970s, John Yudkin proposed that sugar, specifically fructose, was a significant dietary factor contributing to cardiovascular disease. Initially dismissed, Yudkin's hypothesis has gained substantial support over the last 25 years. Emerging research links fructose metabolism to several risk factors for atherosclerosis, including: - Fructose-induced Uricemia - Insulin Resistance - Lipoprotein Hyperproduction - Chronic Inflammation 🔬 A Call for New Research Directions: Yudkin's pioneering work in nutritional science underscores the need to re-evaluate our approach to preventing cardiovascular disease. Future studies must explore these metabolic consequences of fructose more deeply and consider their implications in the broader context of dietary guidelines and public health strategies. 🌍 Moving Forward: As we continue to uncover the complex interplay between diet and cardiovascular health, it’s crucial to integrate these insights into practical, effective dietary recommendations. Recently, nine cellular and #molecular #hallmarks of #ageing have been identified, which characterise the ageing process, and collectively, may be key determinants of the ageing trajectory. These include genomic instability, telomere attrition, epigenetic alterations, loss of proteostasis, deregulated nutrient sensing, mitochondrial dysfunction, cellular senescence, stem cell exhaustion and altered intercellular communication. Healthier dietary patterns reduce the risk of age-related diseases and increase longevity and may influence positively one or more of these hallmarks. The Mediterranean dietary pattern (MedDiet) is a plant-based eating pattern that was typical of countries such as Greece, Spain, and Italy pre-globalisation of the food system and which is associated with better health during ageing. Here we review the potential effects of a MedDiet on each of the nine hallmarks of ageing, and provide evidence that the MedDiet as a whole, or individual elements of this dietary pattern, may influence each hallmark positively—effects which may contribute to the beneficial effects of this dietary pattern on age-related disease risk and longevity. We also highlight potential avenues for future research.
