Here are some steps you can take once you have a list of prospective elderly care facilities for your loved one: "1. Look up each of the listed places and see how they are rated. A "five-star" may be good or not. A "one-star" is sure to be horrible. If at all possible, go visit the proposed facility, unannounced. 2. If you see elders lined up along the walls in wheelchairs, doing nothing, sleeping, that's a bad sign. So is the absence of any staff with them. 3. If you can't go yourself, you can find a care manager or placement specialist to give you the rundown on the facility's reputation. Perhaps for a fee one of them could go visit and see how it looks, reporting back to you. 4. Be sure a bed is available when you need it for your loved one. With the pandemic, more people are in hospitals and nursing homes recovering from Covid. You may not get your first choice. 5. Keep close watch over anyone in a nursing home. If you are far away and can't visit, at least call daily and ask the charge nurse or person caring for your loved one for updates and progress reports. Ask if your aging parent is actually getting the rehab the doctor orders. With staff shortages, they may not be getting it as prescribed and you have to push for it, advocate for your aging parent and hold the home accountable for delivering the care." Who's going to reimagine the nursing home industry? (or already is?) A working paper from the National Bureau of Economic Research is looking at how nursing homes potentially “tunnel” profits, i.e. move them to related parties in order to hide profits. It seems services purchased from related parties are substantially inflated with most of related party spending on real estate & management services. The paper is showing major markups that correspond to crazy hidden profits & many firms substantially understating their profitability. And notice the little slivers spent on actual therapy, nursing, and nutrition. Reading this brought tears to my eyes. My grandmother had a wonderful experience in a local care facility. It was outstanding and she really flurished. 15 years later, my mom broke her hip towards the end of her long Alzheimer's journey. But we were grateful to hear she could be taken to the very same care center, right down the road from us and for which we had such fond memories. But omg...how things have changed. The place is now in disarray. Not enough help, and we were facing a bill of $9k a month! The staff, aides, therapists, and nurses themselves were still amazing- so skilled and compassionate, and doing their best 🙏 - but they were completely understaffed. As we got to be friends, they told us how the center probably needed 3x more staff, and how many were only making $15/hr. We told them how much we were paying and long story short, we ended up bringing my mom home for her last days, hiring some of the aides directly, paying them double, and paying way less for better care at home. Although getting it started will require a great amount of work, I hope the hospitals in Georgia will participate in this program to ensure the nursing home level older adults (55 years does not seem old to me though😂) get the continuity of care they deserve while allowed to stay in their homes. Persons with Medicare and Medicaid could have a longterm care option. “During the height of the pandemic, The national PACE COVID-19 death rate was 3.8%, compared to 11.8% in nursing homes. WelbeHealth — a Menlo Park, California-based PACE organization — was even able to achieve a COVID-19 death rate of 2.4%. “There’s a 24% lower hospitalization rate amongst PACE members compared to dually eligible beneficiaries who receive their care in nursing homes,” Dr. Michael Le, CMO at WelbeHealth, previously told HHCN. “People receiving care through PACE programs are also less likely to be readmitted to the hospital or suffer emergency room visits. All of this significantly reduces the likelihood of being admitted to a nursing home and all the complications that arise from being in a health care setting unnecessarily.” It has been determined from the Department of Community Health the PACE model is well suited to three initial service areas of the state which include: Savannah/Chatham County, Macon/Bibb County and Fulton and Dekalb counties in metro Atlanta. I had the chance to see how this program works during my residency in South Carolina. It is definitely a team approach to comprehensive care. Data has shown not only that it saves lives but also saves healthcare costs by an average of 12%.

But in fact, researchers have found many of these private equity–owned operations, including Lakeview, are funneling funds — almost all of which come from Medicare and Medicaid — to pay exorbitant fees to their affiliated companies… a holding company buys a nursing home and puts its operations in a limited liability company. It then sells or transfers the real estate to another company, owned by the same people, which collects rent from the nursing home. What percentage of CNA time is spent changing residents who are incontinent? What percentage of CNA time is spent changing wet sheets? What percentage of staff time is spent fixing wounds? What percentage of staff time is spent fixing UTI's? When briefs are fully breathable, have inner leak guards, have NO plastic acquisition distribution layer, and are made of soft materials for comfort, residents have more dignity, better clinical outcomes can be achieved, better financial outcomes are possible, and some of the caregiver staffing issues can be reduced. Thinking differently about how incontinence products affect every department within a community may surprise you! (But they do!)
