Re-Opening the Michael Colombini MRI Case Via RCA: https://lnkd.in/eC-k4pHC For those in the healthcare space, they may remember this landmark case. It involved 6-year old Michael Colombini who was getting an MRI as a baseline due to a serious brain surgery he had just survived. Unfortunately, a series of predictable errors queued up in a particular pattern that day, that ended up taking little Michael's life. While this was undoubtedly a horrible tragedy, what could have possibly permitted this to occur? As a result of lessons learned from this incident, has MRI safety improved? If not, why not? Tobias Gilk (International MRI Safety Advocate) and I teamed up to review this case using our #rootcauseanalysis approach to identify the many latent or systemic root causes. We are hopeful this will help to increase awareness of MRI Safety due to such lessons learned, and having others spread such learning. Did You Know? 💭 The significant advancement in electrosurgery came with the development of high frequency alternating current (AC) by William T. Bovie, an American physicist, in the early 20th century. ⚡️ In 1926, Bovie collaborated with Harvey Cushing, a prominent neurosurgeon, to perform the first successful surgical procedure using electrosurgery. The Tengable team was finally able to reunite for an all-hands lunch after spending too much time apart. Here are some highlights 📝: 🔸 In Q4 the team and pace of work almost doubled with the additions of Natalie Zlotnikov and Dana Ben Artzi. 🔸 We share a determination to bring our tactile sense restoration solution to market as quickly as possible––further motivated by the many injured in the current conflict. 🔸 Even though we met to catch up, the conversation quickly turned to technical discussions and executing the roadmap. 🔸 As there is no effective treatment to restore tactile sensation, we understand our responsibility to help the many million victims of industrial accidents, women after a mastectomy, and diabetics with neuropathy, and we are rushing to help. 🔸 Not pictured here is Dr. Amir Arami, our Chief Medical Officer and Head of the Hand Surgery and Microsurgery department at Sheba Medical Center, Tel Hashomer.
