Pharmacy Profitability and Concessions In January of this year Lofepramine Suspension S/F 70mg/5ml 150ml was granted the largest concession so far this year of £147.74. This product has an interesting history. In February 2022 the average market price for Lofepramine Suspension S/F 70mg/5ml 150ml rose from £33.37 to £129.18. This was after a gradual loss of suppliers, dropping from five to just one. This does not seem to be a response to a drop in prescribing because this product still costs the NHS about £200,000 per year. Rather it seems to relate to low pharmacy profitability with suppliers struggling to give any discount at all. The English Drug Tariff rose from £33.87 to £140.07 in March 2022 to cover the market price increase, but the gap between the market price and the reimbursement price soon returns to its previous low profit state. In fact between March 2022 and April 2023 the average market price and the drug tariff were identical, meaning there was no profit in this product for pharmacies. In May 2023 the market price fell, allowing pharmacies a profit till Nov 2023 when price rose again. Charles Joynson comments, 'It does appear as if this is a challenging product to make and distribute. Wholesalers and manufacturers usually do their best to leave profit for their accounts. The DHSC has granted two concessions, the first in Jan 2022 of £104.70 and the second in Jan 2024 of £147.74. However, the number of prescriptions written for this product are gradually declining, this may be as practitioners prescribe more effective drugs with fewer side effects than the older tricyclic antidepressants.' #ukpharmapricing #pharmaindustry

The current onslaught of GLP's may result in dire consequences with long term side effects such as gastric stasis, pancreatitis and even thyroid cancer. Right now your PBM is simply providing a revolving door for approval of the GLP's based on the financial incentives provided by PhRMA in the form of "Class of Trade Discounts" or "Purchasing Contributions" for their mail pharmacies, that goes directly to their profit line. The potential liability due simply to the cost of these and how they affect premiums and potential raises could be the tip of the iceberg if 20 - 30% of your members receiving the GLPs develop these side effects. Please remember the FDA's Phase 4: Phase 4 studies gather additional information about a product's safety, efficacy, or optimal use after approval. Please read the article below. As well, note that GLP-1's are now linked to depression and even suicide!!! Allowing your PBM to approve and dispense these newer and sometimes unsafe medications due to their conflicts of interest from both managing your utilization while dispensing over 80% of your pharmacy spend from their mail and specialty pharmacies where they receive "Class of Trade" financial incentives that are equal to 30% or more of the cost of the medication, puts your company, your executives (personally) and your fiduciary (personally) at risk of litigation and CAA sanctions. More importantly, it can put your employees and their families at risk of severe side effects. It is important to note that these newer, more expensive medications are almost never first line agents.
