Using a tool such as a health information exchange (HIE) is one way to alert home health agencies (and hospice/palliative) of a patient registering at the emergency department (ED) is one way to actively divert patients from an ED visit that may be able to be managed outside of the hospital. But education and 24/7 on call availability as preventative measures are the true first step.
