PCORI approved $207 million to fund new patient-centered #CER studies, including research on #MaternalHealth, adolescent #MentalHealth, #Delirium in older adults, #CardiovascularDisease and more. PCORI also approved more than $10 million to fund implementation projects designed to assess and expand ways to promote the uptake of PCORI-funded patient-centered #CER findings in clinical settings, and nearly $3 million for studies that will strengthen the evidence base on engaging patients and other health care decision makers in designing and conducting patient-centered #CER. Just attended my first Patient Participation Group (PPG) meeting for my local surgeries - Allendale and Haydon Bridge. It’s been great discussing how we can work in collaboration to improve the patient and practice staff experiences, improve communications and improve access for patients. We all know that the #nhs - including #primarycare has many challenges, so it feels like a positive move to volunteer my time to support my local community and our highly valued primary care teams. It was great to meet the other PPG members from the patient population and the Practice, and to share and discuss the challenges the various stakeholders face as well as the numerous opportunities that are available to implement too. It’s definitely not all doom and gloom in Primary Care land, but we as patients do need to play our part too - including recognising and understanding the changes. And, with delivering on #nhsengland Quality Improvement programmes such as the General Practice Improvement Programme (GPIP), I’m hoping that I can put my primary care and quality improvement experience to good use! Looks like there will be a few actions to take forward, but I’m looking forward to engaging with my local community, to gather their insights, experiences and perspectives, and to help communicate the changes within primary care - in particular the resultant changes at our Practice. #volunteering #ppg #qualityimprovement #generalpractice

Just back from the California Association of Health Plans' CalAIM Seminar in Burbank this week and what's stuck with me is the underlying theme within comments made by Dennis Hsieh and Manisha Sharma, MD, FAAFP and others. CalAIM's intent to create a more coordinated, person-centered, and equitable health system that works for all Californians. To get there what has to happen is frontline COLLABORATION. ⇨ Clinics and CBOs should keep pushing MCOs to provide educational and technical support so they can succeed financially and operationally. ⇨ MCOs should be pushing clinics and CBOs to provide the whole-person clinical lens so they can succeed clinically. Recently, I had an interesting conversation with a Pharmacist who had moved to Canada from India. This person had not only cracked through PEBC and got registration in BC but was also a Lawyer. Impressive, right? During our conversation, this person expressed their desire to create whole new advocacy body for the pharmacy profession. They were concerned about certain health professionals monopolizing the industry and pushing back on the expansion of the scope of practice of pharmacists. However, when we suggested including Pharmacy Technicians in the advocacy body, this person wasn't interested and only focused on their own perspective. This situation raises important questions about inclusivity and diversity within advocacy efforts. It's crucial to consider the perspectives and needs of all members of a profession, not just a select few. What are your thoughts on this situation? Their observations highlight increased #engagement among pediatricians compared to medical officers, suggesting the extension of the program to include senior daycare centers for enhanced #impact. The necessity of a systematic approach to connect healthcare professionals, particularly those indirectly involved in thalassemia management, was underscored. #Olderadults with a combination of physical, mental health and social care needs (or #complexneeds) have an increased risk of high health service utilisation. This burdens patients and strains the healthcare system. In this GERI-funded project, the study team examined how a pilot programme for older adults in Singapore that integrates primary care and home care management (integrated #patientcentred medical home (PCMH)) reduced healthcare use. This pilot was part of the Tsao Foundation’s Community for #SuccessfulAgeing (#ComSA) initiative. Compared to controls, participants enrolled in the integrated PCMH for over a year had fewer visits to the A&E, polyclinics and specialist outpatient clinics. This demonstrates the potential benefits of the integrated PCMH care model for older adults with complex needs. Within more deprived areas of the UK, where people have the larger proportion of health needs, General Practice is being increasingly harder to access, due to inequalities in PCN funding and a lack of clinical staff. As the report highlights, "When adjusted for the higher needs of local patients, PCNs in the 20% most deprived areas of England employ six fewer full-time equivalent staff per 100,000 patients than those in the 20% least deprived areas." 2wSuper important, especially with a focus on application (what do I do? How do I do it?). So much of slow adoption is because it’s not clear exactly what to do and no one has your back. Especially a problem where a practical expert is hard to find.
