Today, CMS released the 2024 OPPS Final Rule. The Final Rule includes required schemas for hospitals to post their MRFs as of 7/1/2024. On the heels of these required schemas, we are optimistic about the future of price transparency. Additional prescription drug reporting and, ideally, enforcement dates for AEOBs and GFEs are also on the horizon. I’m energized by the opportunity that Turquoise Health and others have to move the needle on creating a new patient financial experience  – built on a clear, upfront understanding of the cost of care. Another positive step for overall healthcare transparency and a better patient experience! I've had multiple conversations with providers regarding their compliance efforts as we had into 2024. Some have vendors but aren't confident the vendor can keep up with the changes and want to avoid being warned or fined by CMS. Others built their original MRF in-house and would prefer to not go through that time consuming and confusing process again. Many clinical factors contribute to ASCVD, and many interventions exist to help lower the risk. Aside from encouraging patients to change their habits and behaviours (i.e. to stop smoking, eat healthier, incorporate more exercise in their lifestyle), pharmacological interventions mostly involve generic drugs with a low budget impact for healthcare systems. However, there remain patients who develop fatal or non-fatal myocardial infarction or strokes despite treatment, leading to a high burden on healthcare systems. Hence there remains a clinical need for interventions to successfully identify and treat these patients at very high risk to develop ASCVD events. After years of limited new product launches, there are currently a wealth of products in clinical trials, with new modes of action to tackle ASCVD-related conditions. New treatments give hope to physicians and patients alike, to be able to prevent this deadly condition. On a plane to San Diego for 340B Winter Coalition right now! Can’t help but scour the latest news on the way. The blabbering nonsense in this article (not to mention the misleading statistics from the study referenced) perfectly illustrates why we need continued advocacy about 340B. The unsurprising point made here is that payor reimbursement compared to acquisition cost for infusion drugs is higher at 340B hospitals versus non-340B hospitals. Wow, groundbreaking research guys 🙄. You conducted a study to prove the stated mechanism and intent of the program. They falsely equate this to higher drug markups—not what that term means. This was published in the New England Journal of Friggin’ Medicine!!! Obviously NEJM takes drug company money, but the lead author is from Berkeley—I didn’t realize they were so pro-PhRMA. So many points to make here. Just a few that come to mind— 1. Providers don’t generally pass on their exact cost. Why should they? It completely eliminates the incentive to negotiate drug pricing down. And in 340B eligible environments this would mean passing the 340B benefit along to the payor. Not the program intent. And do you suppose they’ll pass it along to the plan sponsor? No way. 2. It’s incorrect to assume everything is acquired at 340B price at a 340B covered entity. For this and many other reasons, chargemasters aren’t based on 340B price. 3. The pharma talking point that 340B is just profit for already affluent hospitals is simply untrue. From the article: “Robinson argued that most of the money is going to mainstream hospitals treating non-indigent patients. ‘This gives them incentives to increase the volume of the drugs they administer…’ “. Nonsense. Plenty of research says otherwise. Ok, now I’m fired up for a great conference!
