Attorneys may find themselves asking, "What kind of nurse expert do I need?" Looking at the inherent issue may assist. Pinpointing if it is department-specific or general nursing care is key. As a nurse expert, I opine on department-specific issues (specifically in the emergency department) as well as general nursing knowledge (fall prevention is a popular one, for example). Specialties may overlap, such as the standards of care seen in conscious sedation, a procedure that may be done in different departments such as emergency departments or surgical centers. Having an emergency nurse expert is helpful, because the emergency department is the door to the hospital. ED nurses do it all, sometimes simultaneously, so our breadth of knowledge is quite large. We care for urgent, non-urgent, critical, traumatic, psychiatric, and surgical patients from neonates to geriatrics. We are highly trained to assist with a plethora of procedures in the emergency department from vaginal examinations to esophagogastoduodenoscopies, and that variety can be effective as a nurse expert. So... any nurse expert can do it, then? 🤔 Publishing and/or speaking on general nursing care helps set the expert apart from other nurses. Safe discharge 👋 This is a common theme I see in my expert reviews. Nurses, even though the provider writes the discharge orders, you are the infamous "eyes and ears" of the department. Making sure your patients are safe to discharge can include (but certainly isn't limited to) the following: - a follow up plan. Instructing the patient who has no PCP to follow up with their PCP just doesn't make sense. - a safe ride home if given medications such as benzodiazepines or narcotics. Even diphenhydramine can make a person too drowsy to drive; make sure the patient has a safe ride home if they're loopy (medical term there 😉). - ensuring the patient can ambulate to their baseline or there is help if unable to ambulate. I'll never forget watching a patient being literally picked up and bodily placed in a wheelchair as she screamed in terror that she couldn't walk due to back pain and leg weakness. After two more ER visits, she was finally diagnosed with cauda equina syndrome and shipped to another facility for surgery. Big oops. - resolution of symptoms. While the ER cannot diagnose and cure every disease, serious and life-threatening diseases should be ruled out before discharging a patient. Treatment to ameliorate symptoms may be attempted. Reevaluate those treatments before sending the patient home, and get those pesky discharge vital signs! - discharge instructions should make sense. This goes hand-in-hand with the above tip. If the symptom isn't resolved, then discharge instructions stating to return to the ER with that very symptom just don't make sense to the average patient. - have your patient do a teach-back to ensure their understanding of the discharge plan. This is the easiest way to gauge how much they received of the discharge teaching spewed at them. - make sure the discharge instructions are in the correct language. The list isn't comprehensive, and ER nurses know well-enough that some people just won't be happy with their care. Hospital #nurse: When out in public, rather than refer to yourself as a “floor nurse” or “staff nurse” use “clinical nurse” and mention what type of department you practice in. Give examples of the types of patients you care for. Example: When asked what you do you might say, "I'm a registered nurse. I practice as a clinical nurse on the neurology unit. I care for people who have had strokes and brain trauma." This is more descriptive and less generic. If you're a case manager (or other position where 'nurse' might not automatically be part of the title) be sure to introduce yourself as a 'registered nurse case manager' and in a sentence describe what you do. There is power in words. This is one way to advocate for yourself and the profession as well as promote the diversity and depth of our role. #NursesOnLinkedIn #NursePower #nurse
