For July's #DDU journal club we're highlighting a recent #jmedchem drug annotation describing the development and optimisation of the clinical candidate Sonrotoclax for B-cell malignancies. Sonrotoclax is a protein-protein inhibitor of Bcl-2, a key mediator of cellular apoptosis. It prevents Bcl-2 from sequestering proapoptotic proteins, promoting apoptosis of cancerous cells. The authors used co-crystal structures to design molecules with a novel binding mode in the P2 pocket, distinct to previously disclosed inhibitors, including the clinical approved Bcl-2 inhibitor Venetoclax. We are excited to announce initial Phase I data of JBI-802 a CoREST inhibitor, which shows therapeutic potential in immunotherapy resistant NSCLC patients with poor prognosis based on their genetic and metastatic pattern. In addition, the on-target dose/exposure-proportional decrease in platelet constitute a proof-of-principle that JBI-802 can be an active compound in hematological malignancies like Essential Thrombocythemia (ET) and other MPN/ MDS characterized by thrombocytosis. A follow up Phase I/II study in MPN/ET and MPN/MDS with thrombocytosis is being planned in first quarter of this year to investigate JBI-802 as potential novel treatment option. 𝐂𝐚𝐥𝐥𝐢𝐝𝐢𝐭𝐚𝐬 𝐀𝐧𝐧𝐨𝐮𝐧𝐜𝐞𝐬 𝐏𝐨𝐬𝐢𝐭𝐢𝐯𝐞 𝐃𝐚𝐭𝐚 𝐟𝐨𝐫 𝐒𝐞𝐭𝐚𝐧𝐚𝐱𝐢𝐛 𝐢𝐧 𝐒𝐂𝐂𝐇𝐍 𝐓𝐫𝐢𝐚𝐥 Calliditas Therapeutics AB (Nasdaq: CALT, Nasdaq Stockholm: CALTX) Calliditas Therapeutics announced positive data from the Phase 2 trial of setanaxib in combination with pembrolizumab for squamous cell carcinoma of the head and neck (SCCHN). The trial showed significant improvements in progression-free survival (PFS) and overall survival (OS), along with changes in tumor biology consistent with setanaxib's mechanism of action. The trial involved 55 patients with recurrent or metastatic SCCHN. Patients receiving pembrolizumab and setanaxib demonstrated significant improvements in key secondary endpoints compared to those receiving pembrolizumab and placebo. Transcriptomic analysis showed an increase in CD8+ T-cells in setanaxib-treated patients. "The results provide a strong basis for advancing setanaxib," said Professor Kevin Harrington. CEO Renée Aguiar-Lucander Renee Aguiar-Lucander expressed excitement about the positive clinical evidence supporting their NOX platform. CMO Richard Philipson Richard Philipson thanked the trial participants and investigators for their contributions. Calliditas is conducting additional trials with setanaxib, with readouts expected in Q3 and Q4 of 2024 and 1H 2025. The field of radiotheranostics, which combines diagnostic imaging and targeted radiotherapy, has seen significant advancements with several promising agents being explored for precision oncology. Lutetium-177-PSMA-617 is being used for metastatic castration-resistant prostate cancer, showcasing the potential of targeted therapy against prostate-specific membrane antigen (PSMA) to manage advanced stages of the disease. Yttrium-90-labelled ibritumomab tiuxetan and Iodine-131 tositumomab (Bexxar) are examples of radioimmunotherapy approaches for non-Hodgkin’s lymphoma, leveraging the targeting capability of antibodies conjugated with radioactive isotopes to deliver cytotoxic radiation directly to tumour cells. Radium-223 represents a breakthrough for treating metastatic prostate cancer, specifically targeting bone metastases and improving survival rates. These advancements highlight the growing role of radiotheranostics in oncology, offering more precise and personalised treatment options for cancer patients.
