Meet Mr. C! When Mr. C first arrived, he needed full assistance with all care. Before his hospitalization, he managed his self-care in a wheelchair but couldn't walk due to pain from his prosthesis. During his rehab stay, our therapy team worked with his prosthetist to adjust his prosthesis, making standing tasks tolerable. The results? Incredible progress! 🔥A Key Ankle and General Lower Quarter Assessment🔥 The Lateral Tibial Glide is a quick and easy assessment that help check ability to load the lower quarter. I use this in conjuction with lumbar, hip, knee, and half kneel ankle df. Regardless of the condition, as a pattern you will see a lack of lateral tibial glide on the involved side, along with a deviation away in double limb squat and inefficient loading or what looks like instability in single limb squat. Check this especially with hip impingement or runners with knee pain, pain with squat and also unilateral lumbar/SIJ complaints. So check out this ACL tear. This patient is a Division 1 athlete. She has an avulsed ACL tear off the femoral wall. Check out how robust the ACL still is... do you really want to throw that away?? Not this Fro, the ACL stump and remnant is well vascularized and has proprioceptive nerves. We repaired the ACL and will now place the Miach Orthopaedics, Inc. BEAR implant over top of the native ACL. The data for repair and augmentation is growing everyday. This patient will not require injury to her quad, patella, or hamstring tendons. She will be off crutches and a brace at 4 weeks. Most patients undergoing this procedure in my practice have less than 5% quad atrophy. 🫶Embracing an Orthodontic Recovery Plan🫶 Sometimes things dont always go to plan and you need to rethink strategy to overcome complications that can arise during orthodontic treatment. Our patient started treatment as a teenager with another orthodontic clinic. Four years of treatment was not working out 🅦🅔 🅝🅔🅔🅓 🅐 🅝🅔🅦 🅟🅛🅐🅝 The plan was to align her teeth and align the impacted upper canine (UR3). Initially things were going to plan, but after some time it was clear things were not happening as planned. The UR3 was no longer moving! - The UR3 was ankylosed (fused to the bone) - Instead of the canine erupting the entire upper arch started to intrude (lift up wards) leaving an extensive open bite and reducing the overall amount of tooth visible on smiling. When things don’t go to plan it’s important to pause - rethink - and recreate a new strategy and a 🅝🅔🅦 🅟🅛🅐🅝 This takes time for any clinician not least when you take over another colleague’s treatment but also patients need time to understand their complications and process the treatment options and limitations.
