New publication! "Understanding Patients’ Preferences and Experiences During an Electronic Health Record Transition" in Journal of General Internal Medicine with Gemmae Fix, Alison Cogan, Stephanie Leah Shimada, Jessica Davila. Using multiple qual. methods, we explore how patients have been affected by the VA's transition to a new electronic health record. Our findings address communications about the transition, enforcing trustworthiness, and ways to guide patients using the new portal. Maintaining clear communication across patients, local leadership, and providers throughout an EHR transition is essential. Patient-facing communications can set expectations, and help patients receive adequate support, particularly related to the patient portal.

Very excited to share with you all our recent publication looking at lessons learned from implementation of an electronic health record in a LMIC. As you can appreciate, implementation of this key tool can be drastically different in resource limited countries compared to western nations. Reflecting on my significant experiences with EHR implementations, I recall working extensively with the Vista platform in private hospitals for many years. Vista had its merits, offering more stability and functionality than some of the newer systems being introduced today. It's worth noting that the VA pioneered EHRs using Vista long before many others. Given the legacy and infrastructure already in place, perhaps revisiting and enhancing the Vista platform would be a prudent cost-effective move. After all, industry giants like Epic operate on the MUMPS platform. What's stopping the VA from leveraging the potential of Vista? Like many Australians I and members of my family have auto-immune diseases which require continuous blood tests, radiology and long term medications to manage. One of the diseases in my family is a rare form of RA which requires multiple specialists and imaging on a young person. It infuriates me that Australians can’t easily get access to these constant interactions with different parts of the health system and often imaging has to be repeated for the convenience of the specialist with no thought to the impact on a growing young body. This is the impact of silos and a lack of interoperability and powerful pathology and imaging lobby groups who don’t want their revenue streams impacted by the ability for the consumers and other providers in the care team to easily access their results. It’s not good enough and it urgently needs to change. 'A patient-owned record is even more important as patients build up more and more important data in the 21st century from home devices. No longer is the GP’s record “the record”, only the patient’s own record - combining data from all sources including the home - can be “the record”. 'Patients who can self-assess and self-manage are the only path forward with an overworked workforce and underfunded funding model. Not everyone can do everything but many can do much, and we need them to free up resources for the most vulnerable.'

But professional resistance and policy inertia have proved difficult to overcome. It's encouraging to know that efforts are being made to find solutions like AI and Remote monitoring. Much of what we see from the media is negative negative negative.... Also, in addition to physicians, I've seen firsthand the significant shortage of physical therapists across the US, which underscores the urgency of these efforts. Thanks for sharing!
