I had the distinct pleasure to be invited for an interview by Richard Jacobs on Finding Genius Podcast USA show on The Florey sepsis research program: MEGA-SCORES In this interview, we discussed the following: 1. How sepsis is a global healthcare problem and the urgent need for developing new effective therapies. 2. The complex pathophysiology of sepsis-induced multi-organ dysfunction that complicates management of patients in intensive care units. 3. As we move through the last two weeks of Stroke Awareness Month, I am amazed when reflecting on the many developments that have occurred in basic science, medical devices and pharmaceuticals, and healthcare delivery over the last 15 years in the neurovascular health care space. However, despite numerous transformational advancements there are still significant variations in stroke care that lead to disparities in outcomes. Addressing disparities in stroke care is about more than merely discussing race, ethnicity, and other social determinants of care. On a larger scale it is an exercise in attempting to understand why many examples of variations in healthcare occur.  This refection leaves two strategic questions to ponder: 1)     How can health care systems utilize strategy and leverage data to solve complex medical problems in a way that adds value to both the patients and the healthcare ecosystem as a whole? 2)     How can health care systems, corporations, academic institutions, and financial institutions collaborate to identify effective and fiscally sustainable solutions to solve healthcare's biggest problems? Complex issues in healthcare will continue to require strategic collaboration, data-driven solutions, and engagement at multiple nodes along the continuum of care and this is particularly true for stroke care. I had the opportunity to collaborate with Shelly Ikeme, her team of physician colleagues, and the CERENOVUS division at Johnson & Johnson MedTech on work that attempts to unpack some of these ideas. My article regarding neovascularisation of the eye has recently been published in the Klarity health library. Here I described the causes of neovascularisation, including inflammation, ischaemia and underlying conditions, as well as exploring the mechanisms and manifestations of this form of angiogenesis. Excited to share our new paper published in Stroke on expanding access to mechanical thrombectomy, a life-saving stroke surgery. Despite proven benefits, gaps remain globally. I was honored to collaborate with esteemed colleagues to analyze challenges and solutions to improving thrombectomy implementation. Please help share this important cause! Read the full article at . https://lnkd.in/dj8hW6da ✅Main points from the article "Challenges to widespread implementation of stroke thrombectomy":✅ ❎- Endovascular treatment (EVT) for acute ischemic stroke is highly effective, but implementation lags behind in many areas. ❎- Patterns of EVT underutilization exist across healthcare systems and include: 1) Complete lack of access in some low- and middle-income countries (LMICs) 2) Geographic disparities in access within a country, with lack of access in rural areas 3) Social/ethnic disparities, with certain groups less likely to receive EVT 4) Time-dependent disparities, with lack of 24/7 access to EVT services ❎- Overcoming complete lack of access in LMICs requires building EVT infrastructure from the ground up. This is low priority given limited healthcare budgets. ❎- Reducing geographic disparities may involve telemedicine, transport protocols, and simulation training. ❎- Mitigating social/ethnic disparities requires addressing biases, financial limitations, and improving stroke awareness. ❎- Expanding 24/7 access requires sufficient trained staff. Time-based disparities are a common, temporary issue when establishing new EVT services. ❎- Implementation research is needed to study barriers to EVT uptake. Strategic partnerships and lobbying can also facilitate EVT implementation. How it could be useful to administrators and policy makers? In summary, the article provides evidence, analysis, and guidance to inform health administrators and policy makers seeking to improve access to this highly effective stroke treatment. It highlights the value of coordinated efforts across stakeholders to address implementation gaps.
