Principles of aseptic techniques in OR room for radiographic technologist: • Only sterile items are used within the sterile field. • Only sterile persons handle sterile items or touch sterile areas. • Nonsterile persons touch only nonsterile items or areas. • Movement within or around a sterile field must not contaminate the sterile field. • Items of doubtful sterility must be considered nonsterile. • When a sterile barrier is permeated, it must be considered contaminated. • Sterile gowns are considered sterile in front from the shoulder to the level of the sterile field and at the sleeves from the elbow to the cuff. • Tables are sterile only at table level. • Radiographers should not walk between two sterile fields if possible. • Radiographers should avoid turning their backs toward the sterile field in compromised spaces. • The radiographer should watch the front of clothing when it is necessary to be next to the patient. • The radiographer must be aware of machinery close to the sterile field, including lead aprons hanging from the portable machine that may swing toward the sterile field. • The lead apron needs to be secured if it is being worn next to the sterile field. • The apron can easily slip forward when raising one’s arms up to position the tube. A properly worn apron does not compromise the sterile field or jeopardize proper body mechanics. • When positioning an IR under the OR table, the radiographer should not lift the sterile drapes above table level because this would compromise the sterile field.

Conversation between 3 gps regarding the role of medical consult in the real world Doc 1 What should you do if you completely disagree with a recent medical consult you received back from a patient's cardiologist. (Why did you send pt for m.c. in the first place) Have a patient on blood thinners and 6 months status post prosthetic heart valve replacement. Cardiologist wants no premedication and to discontinue Blood thinner for Sc/Rp. Doc 2 I don't ask the MD permission to do dentistry as they don't have a clue what I am doing. Doc 3 If you knew what you wanted to do why did you ask the MD in the first place? Doc 1 I was just hoping to cover my butt by having the cardiologist validate what I was thinking. Me: 1. Nothing will happen to your patient regardless what kind of dental procedure it is; 2. In the extremely unlikely event that if anything did happen, no medical consult from M.D. will "cover my butt ".

Batching tasks is truly crucial during our busy workload as surgeons. I've found it incredibly productive, helping me achieve a more organized day with less pressure. Here’s what I do 👇( every surgeon should try this; it will be life-changing for you.) 1. Pre-Operation Preparation: When I have a theatre list, I start early by seeing the patients, obtaining their consents, and drafting the operative notes. This preparation ensures that everything is ready while I am busy between operations, making the whole process stress-free. 2. Clinic Management: In the clinic, I see all the patients first and quickly write in their notes, documenting the diagnosis and plan. After clearing all the patients, I dictate the clinic letters from the notes all at once. This way of batching similar tasks together has been very effective and significantly reduces tension. 3. General Task Batching: I apply the same concept to every aspect of my workload. By batching similar tasks together, your mind will become more efficient at completing them in less time. For example, I set specific times for replying to emails and writing necessary letters or any written content. 👉I also use this method for creating my online content and writing my newsletter. I allocate a specific time to write all my content for the whole week and schedule it, so I don’t have to worry about it during my busy week as a surgeon. You have to effectively start applying batching tasks to your daily jobs.
