My latest column aims to educate caregivers of sickle cell patients, as their role is crucial in improving the patients' quality of life. The level of caregiving provided can significantly impact the health outcomes of individuals with sickle cell disease. In my article titled “How to be a better caregiver for sickle cell patients.” I provide caregivers with valuable tips to improve their skills. Here's an excerpt from the column: “In my years with sickle cell, I’ve relied on the support of dedicated caregivers who’ve helped me navigate complications. My mother, sisters, father, aunt, and friends have shown me endless care and support, which is why I’m still here today. Following are some of the ways my caregivers have enhanced their skills to help me. Caregiver support techniques For one, they constantly educated themselves about the disease to better understand it and help me manage my needs. In fact, all sickle cell caregivers should familiarize themselves with the disease’s symptoms, pain crises, complications, prevention, and treatments. The most recent "Proposed" LCD changes from CMS could potentially handcuff us in one of the most important times in Advanced Wound Care. We, Ensite Medical, are currently working on "The Diabetes and Wound Care Initiative for Native Americans". Never before in history have not only Native Americans but all Americans had the access to Advanced Wound Care options that are available today. Ensite Medical's ultimate goals for Oklahoma is to decrease the amputation rate within Native Americans by 30% or more over the next 3 years. This is done several ways, including by educating patients about diabetes and vascular disease and by educating physicians and medical facilities about Advanced Wound Care options. However, in states like Oklahoma we have a large rural population with socioeconomic challenges. Patients are not only challenged by the cost of medical care but access to medical care. Many wounds require daily and weekly treatments. This is where Mobile Wound Care comes in, and thank goodness we have Mobile Wound Care teams available in Oklahoma. Mobile Wound Care is NOT the same as Home Health. Mobile Wound Care Providers following these patients closely and are spending alot of time and money on the road, going to these patient's homes. Skin substitutes, mobile O2 Therapy (Like Natrox), wound care kits, medical supplies, mobile wound care companies... The list goes on. Please do not handcuff these providers by taking away the options that they have recently been made available them to treat patients. Don't leave these patients hopeless. Limb salvage is real. 🏥🏠 Mayo Clinic's innovative "Hospital at Home" program is redefining patient care! Since its launch in 2020, it has treated over 6,100 patients and saved more than 15,000 hospital bed days. 📈 Initially focusing on conditions like CHF, COPD, and pneumonia, the program now includes post-surgical and bone marrow transplant patients. The key? Patients must be hospital-level sick but stable enough for home care, avoiding the need for ICU or immediate surgeries. 👨⚕️🛌 Dr. Michael Maniaci, MD, the enterprise physician lead for this program, shares intriguing insights. Home care patients, he notes, recover faster, requiring fewer post-acute services. Patients in the home hospital program have an illness severity of 2.8, which is higher than the average acuity of patients in the hospitals being covered. This isn’t just glorified home health. 💡 The results are impressive: lower readmission rates, excellent patient experiences (90% enrollees recommend it), and potential cost savings of 5-10% for hospitals. Do you remember what it was like in COVID-19 when family caregivers were locked out of hospitals, clinics, group homes, supportive living, and long-term care? It wasn't good for patients, caregivers, or healthcare teams! One caregiver told us about not being able to accompany her husband with dementia, diabetes, and a wound on his foot, "I am supposed to take care of this, but he doesn't remember what they told him." Today's shout-out is to Alberta Health Services and their Family Presence Policy, in which they recognized family caregivers' essential role. Weren't they one of the earliest health services to do so? Systems of care have been developed across the United States to standardize care processes and improve outcomes in patients with ST-segment–elevation myocardial infarction (STEMI). The effect of contemporary STEMI systems of care on racial and ethnic disparities in achievement of time-to-treatment goals and mortality in STEMI is uncertain. We analyzed 178 062 patients with STEMI (52 293 women and 125 769 men) enrolled in the American Heart Association Get With The Guidelines–Coronary Artery Disease registry between January 1, 2015, and December 31, 2021. Patients were stratified into and outcomes compared among 3 racial and ethnic groups: non-Hispanic White, Hispanic White, and Black. The primary outcomes were the proportions of patients achieving the following STEMI process metrics: prehospital ECG obtained by emergency medical services; hospital arrival to ECG obtained within 10 minutes for patients not transported by emergency medical services; arrival-to–percutaneous coronary intervention time within 90 minutes; and first medical contact–to-device time within 90 minutes. A secondary outcome was in-hospital mortality. Analyses were performed separately in women and men, and all outcomes were adjusted for age, comorbidities, acuity of presentation, insurance status, and socioeconomic status measured by social vulnerability index based on patients’ county of residence. Compared with non-Hispanic White patients with STEMI, Hispanic White patients and Black patients had lower odds of receiving a prehospital ECG and achieving targets for door-to-ECG, door-to-device, and first medical contact–to-device times. These racial disparities in treatment goals were observed in both women and men, and persisted in most cases after multivariable adjustment. Compared with non-Hispanic White women, Hispanic White women had higher adjusted in-hospital mortality (odds ratio, 1.39 [95% CI, 1.12–1.72]), whereas Black women did not (odds ratio, 0.88 [95% CI, 0.74–1.03]). Compared with non-Hispanic White men, adjusted in-hospital mortality was similar in Hispanic White men (odds ratio, 0.99 [95% CI, 0.82–1.18]) and Black men (odds ratio, 0.96 [95% CI, 0.85–1.09]). Race- or ethnicity-based disparities persist in STEMI process metrics in both women and men, and mortality differences are observed in Hispanic White compared with non-Hispanic White women. Further research is essential to evolve systems of care to mitigate racial differences in STEMI outcomes. 💥💡The increasing complexity of inpatients is not only a perception, it is real, as recently shown in this interesting Canadian study including 3.4 million patients during 15 years. It’s expected in the context of an older population, better hospital and ICU protocols, more advanced cancer treatments, etc., which improves survival of patients who couldn’t have survived 10 years ago. However, inhospital resources and staffing have not increased in parallel with patient complexity, which leads to a big challenge for healthcare systems to maintain high standards of quality of care and preventing physician’s burn out.
