In the ever-evolving landscape of healthcare, the role of FQHC Primary Care Associations (PCAs) has become increasingly pivotal, writes our Head of External Affairs Loren Anthes, MBA, CSSGB. With greater pressure from managed care organizations and the government to move to value-based care, PCAs are often tasked with identifying solutions that enable their members to take advantage of these opportunities — while ensuring they aren’t taking on unnecessary risk or making costly investments that often take years to develop. Our #PCnetwork's vision is for clear, cohesive national contracts for practices and PCNs which ensure patients receive equitable care no matter where they live, while enabling greater local commissioning and provision based on the needs of local populations and individuals. This approach could lead to the alignment of national contracts, health inequalities funding and local incentives to deliver for patients according to their specific needs, promote innovation and service development at scale. In the network's report 'Supporting general practice at scale: fit for 2024/25 and beyond,' published last year, we called for a primary care investment standard. NACHC President and CEO Kyu Rhee, MD, MPP, on the short-term funding fix passed by Congress that extends funding for health centers and health care workforce programs through early 2024: “While we join our members in breathing a sigh of relief that Congress did not allow Community Health Center funding to expire on November 17, the repeated short-term funding extensions are disrupting the work of health centers to care for 1 in 11 Americans. 📣 Check out Build Healthy Places Network NEW Community Close-Up! 👉 Hospital closure is one of the biggest challenges that many rural communities face leaving residents unable to receive access to health care. Mackinac Straits Health System (MSHS) located in Michigan’s upper peninsula struggled to remain open and had limited physician resources. Instead of closing the doors of another rural hospital, MSHS forged a strategic partnership with The Sault Ste. Marie Tribe of Chippewa Indians, a federally recognized American Indian Tribe with around 50,000 members. It offers several programs and services, including critical health care, but many Tribal members are unable to access these services due to long distance travel to the Tribe’s major health centers to receive care. And as promised in the CMS Innovation Center Strategy Refresh, https://lnkd.in/e_jdpt-8, the new model incorporates building capacity for #socialrisk screening and #socialneeds interventions. Critical to all these models is data collection and #interoperable data exchange. "Through AHEAD, CMS aims to strengthen primary care, improve care coordination for people with Medicare and Medicaid, and increase screening and referrals to community resources like housing and transportation to address social drivers of health."

Big news today: Building on a state-led model advanced by the Center and Milbank Memorial Fund, Centers for Medicare & Medicaid Services has launched the new AHEAD Model to address systemic drivers of unsustainable health care costs and fragmented care. This model will provide resources and accountability for states to drive transformations in quality, equity and affordability.
