🎙️ GENERAL ELECTION 2024: Week one reflections 🎙️ We’re over a week into the #election campaign and it’s been a fast start. We’ve had multiple gaffes to lighten the mood. Some unforced – e.g. the PM spending one of his first visits at Belfast’s Titanic Quarter, with all the ‘sinking ship’ symbolism that brings, or Ed Davey falling off a paddleboard into a mouthful of sewage... But we’ve also had a rash of policy announcements, so what do the early forays of the campaign mean for health? 💸 While the NHS hasn’t been the dominant issue – this has been more about the economy and tax pledges – we've had some interventions on health from the main parties. 🎯 Labour were first out the traps on Wednesday with a focus on reducing elective waiting lists, at a cost of £1.3bn in the first year. This would pay for 40,000 extra appointments, scans and operations a week, including during evenings and weekends (in part, by using spare capacity in the private sector). If successful, by the end of the next Parliament Labour would meet the target of treating 92% of patients within 18 weeks of referral. The target hasn’t been met since 2016 and, as it stands, 3.2 million of the 7.5 million waiting list have been waiting more than 18 weeks. 🆕 This announcement was in contrast to the Conservative Party’s first outing on health, which focused on boosting primary and community care. It was refreshing to see services that are often overlooked come centre stage, and this is the right direction of travel as we set out in the NHS Confederation manifesto. But successive governments have set out this ambition before and rarely delivered. ✂️ There is a danger we won’t have the staff to deliver on this given, as the NHS Confederation survey from last week revealed, many NHS orgs including in primary care are having to make short term cuts to their staffing levels to balance the books. ❌ Also, a word of caution on the management cuts proposed by the Conservatives. We know this plays out well in public opinion polling, but there is a gap between perceptions and reality when it comes to the number of managers working in the NHS, and wider understanding of the role they play in reducing pressure on clinicians and in improving productivity. 🆕 Finally, it was good to see Lib Dems open up with a focus on the public health grant, which has been slashed over the last decade. As we said in our response, the 28% real-terms per person cut in the grant since 2015 has reduced vital services such as drug and alcohol support for young people, smoking cessation and sexual health services. 🔜 Each party is likely to publish their manifestoes within the next 1-2 weeks, and NHS Confederation will examine what their proposals mean for health and care. I wrote this piece published in March, with thanks to SPERI, on why the NHS is in crisis. Looking at three main areas —  the pandemic, ⁠political dynamics since 2010, and the underlying societal issues holding the service back. Each of which raises vital and distinct aims for public policy;  and for us all who want to see the NHS back on its feet, and universal healthcare free at the point of use secure and flourishing again in the UK. https://lnkd.in/gw-DpTrn

Interesting 🧐 data from the Nuffield Trust: This analysis shows that despite the often repeated ambition to shift more care and health care resources into services “closer to home” – to keep patients out of hospital, to better manage long-term conditions and to boost primary and secondary prevention – funding for the NHS services that primarily provide those forms of care (NHS community care services, primary care and local authority public health services) grew in real terms by less than half a percent on average a year between 2016/17 and 2022/23. Perhaps even more strikingly, funding for NHS community health care services was cut in real terms in three out of the six years between 2016/17 and 2022/23.

The provocative title of this article is not even the worst thing about it. It's very easy to focus on the news headlines about historic vacancies and record-long A&E waits. These are important and they should be addressed, but it's still essential to acknowledge the fact that the NHS (despite its structural problems) is still delivering high-quality care to thousands of patients per day that would not be able to access such care under different governments.
