The Office of the National Coordinator for Health Information Technology (ONC) is seeking public comments on its draft 2030 Federal Health IT Strategic Plan through May 28. In a recent Health IT Buzz blog, ONC wrote that the draft plan builds upon the progress of the 2020-2025 plan, which promoted a modern #HIT infrastructure and advanced the access, exchange, and use of #EHI. The proposed plan also has an increased emphasis on areas such as public health, health equity, and #AI. Respecting Autonomy in/and Digital Health service delivery models 📈 While research publications continue to grow exponentially in the digital health space, authors note  that for success, the evolving connected health delivery model must be driven by health equity, focused on people at the greatest risk for adverse health outcomes, simultaneously improving individual and population health and use of system resources, designed, studied, and implemented across disciplines and parts of the health continuum, including linking primary, community and specialist care around patient-defined outcomes, and fundamentally designed for person-centred care, focused on self-management, patient autonomy, and close relationships with providers. Digital outreach requires that situational awareness, capacity building, and cultural safety are addressed as essential components in the proposition of innovative approaches and technology that aspires to improve health care delivery in these settings. Addressing these factors and to focus realistically and meaningfully on digital health service delivery models means going beyond the traditional biomedical definition of health to better appreciate factors affecting the broader social determinants of health. 1️⃣ Importantly, how these impact the space of innovation, data and care delivery between traditional polarities of health systems to include autonomy and the ecology of chronic disease interactions. If not, we will inevitably widen the digital divide. Building trust should also go beyond bilateral negotiations and agreements, recognizing that [digital] relationships are not political or academic ventures, areas that are often fraught with the exploitation of data. 2️⃣ Eliminating the digital divide requires developing access and ownership policies related to data collection from which communities can become empowered. We need to better understand the sustainability factors impacting these, as they are defined at population level and cannot be fragmented at the peril of meaningful digital health. Sustainable change will be the result of actively listening to communities to understand their needs and develop infrastructure that local leadership can manage. Stipulating how these goals are to be met allows for true collaborative efforts to unfold with technologies useful to these communities. A great discussion illustrating why our health policy teams and digital health teams need to be working closely together to redesign our health system if interoperability is to deliver health equity . Interoperability is just part of the tool kit . Is this collaboration happening in Australia ? #healthequity #dataliquidity ‘We have such limited data in an interoperability environment that we tend to look for clusters and focus on that. Interoperability exposes data silos, and data deserts, where there isn't any data to be found. If you don't have the right data flowing through this, the interoperability value proposition becomes more difficult to execute." Rethinking which data can and should be interoperable is a part of addressing health inequity, said Fillmore. "Think back to the pandemic," he said. "In some states, there were real-time dashboards regarding what beds were available, etc. That's interoperability too. It's not just having data flowing; it's having bed counts. Think of what a real-time dashboard on dialysis could do." Since having the right data can help identify the needs of these underserved communities, there are potential positive implications for public health and maternal health, and several other historically marginalized subcategories.

WHO Regional Office for Europe has recently launched the Strategic Partners’ Initiative for Data and Digital Health (SPI-DDH), bringing together 53 Member States and leading organizations to advance digital health solutions. This initiative aims to tackle the challenges of #digitaltransformation by focusing on safe, affordable, and #patient-centered technologies. Through multisectoral collaboration, SPI-DDH is expected to form four working groups that will focus on enhancing capacity and #access to care using #digitalization and #AI, promoting #homecare, improving #interoperability, and strengthening #publichealth systems. Engage Co-Design Partners in Implementation Design Use co-design partner insights to plan for how the digital health solution should be implemented, including how users will perceive a need for the solution, seek the solution, reach the solution, and adopt the solution 5. Co-Design Equity-Informed Data Collection and Tailoring Use co-design partner insights to identify equity-informed data collection measures that can be used to create an adaptable solution that will meet users' needs over time 6.
