It's #FACEFriday! Let's celebrate the F.A.C.E.s of HSO this week: Christie Roberts, LPN, and Laura Deck, C-MA, at HSO Wilmington. Laura Smith, C-FNP saw an elderly patient with very limited mobility for wound dressings in the office. By the time the patient made it out to the car and then struggled to enter the car, her dressing had come loose. Instead of having the patient try to get back out of the car and into a wheelchair both Christie and Laura went out to her. They assisted her into the car as well as redressing her wounds while she sat inside her own vehicle. Time to tie the knot and make a commitment to making primary care PRIMARY again? Shouldn't we be focusing on restoring and strengthening the relationship between the patient, their family caregiver and the primary care team? Healthcare based on a transaction, disconnected from primary care simply fuels our longstanding fragmentation syndrome and does not work. In the good old days, people looked to their family doctor, who was trusted and who knew everything about them and their health. The entire family was engaged in making decisions and following through with the care that needed to happen. So, do we long enough for the 'good old days' to do something to bring them back? When will we 'tie the knot' and 'recommit' our healthcare system to one that reconnects, restores and strengthens the trusted primary care, patient and family caregiver relationship so that continuity of care becomes the 'norm'...and with it better health outcomes?
