Today, a case about mifepristone (one of the medications commonly used in medication abortion) was argued in front of the Supreme Court. First: if you are seeking abortion care now, nothing has changed so you should keep your appointments and plans for your care. Abortion providers across the country (including carafem) can still serve you in the majority of US states with both in-person and telehealth care. Secondly, no matter what arguments are being made among a small number of non-medical experts, decades of data and millions of people who have taken the medication agree: mifepristone is safe and effective. One thing is certain: carafem will continue to innovate and provide abortion care in the most accessible ways possible for decades to come. The Supreme Court just announced they are going to take a case about mifepristone, one of the pills involved in medication abortion, this term. This means they will rule on whether or not to limit access to a safe and effective, decades-long FDA-approved medication, but for now, nothing changes. This news will be confusing and worrying to people who need abortions right now. Over 750 medical professionals recently called on MPs to back an amendment to the Government’s Criminal Justice Bill, in what would be the biggest change to abortion law for a generation. In a letter to all 650 MPs, the medical professionals urged MPs to vote in support of the landmark amendment tabled by a cross-party group of over 30 MPs, led by Caroline Ansell MP, that would lower the abortion time limit from 24 to 22 weeks in line with advances in medical science. A 24-week abortion limit is now beyond the point when many babies survive, double that of the most common time limit among European Union countries and represents a contradiction at the heart of our abortion law. Originally set at 28 weeks, the abortion limit was lowered in 1990 to 24 weeks gestation. Improved survival rates for extremely premature babies between 24 and 28 weeks was one of the key considerations that motivated this change. By the same logic, and informed by the improved survival rates for babies born at 22 and 23 weeks gestation, MPs are now calling for the abortion time limit to be updated.

Earlier this week, the Texas Supreme Court overturned a lower court order that would have allowed a Texas woman — who was more than 20-weeks pregnant with a fetus diagnosed with a fatal condition — to get an abortion in the state. Kate Cox’s physician believed her circumstances qualified for an exception. However, like many physicians in states with abortion bans, her physician and the hospital did not want to risk criminal and professional penalties by providing an abortion, so they sought a court order. Our 2023 National OBGYN survey found that over half of OBGYNs practicing in states where abortion is banned reported being concerned about their legal risk when making decisions about patient care and the necessity for abortions. The risk of criminal prosecution and penalties facing doctors is so high that many are hesitant to provide or even refer to life-saving abortion care. Abortion reporting following changes in state legislation - Reporting of abortion complication before and after state-mandated reporting changes Monthly state abortion complication reporting before (Pre) and after (Post) state-mandated reporting changes. Institutional complication indicates an abortion complication report submitted for patient that received a termination at our facility (reporting for termination following previable rupture of membranes was not performed before 2021 legislation). All terminations at our institution before the new legislation were performed up to 20 weeks postfertilization for maternal life-threatening indications or for life-limiting fetal anomalies. After legislation, all terminations were performed for maternal life-threatening indications; termination for fetal indications is no longer permissible by law. Today is Abortion Provider Appreciation Day! Abortion providers are crucial to protecting reproductive freedom in the US, especially as we face unprecedented challenges to abortion access post-Dobbs. Providers nationwide are going above and beyond to ensure that patients can make health care decisions with dignity and autonomy. Despite coercive policies, stigma and other unjustified threats, providers show up every day to give compassionate and empathetic care. Data from our Monthly Abortion Provision Study show that more people in the US are being forced to travel out of state for essential abortion care due to abortion bans and restrictions. That’s unacceptable. Policymakers must step up to support abortion providers in local communities. This includes expanding and strengthening the provider pool and ensuring financial support for clinics.
