On a monthly basis we partner with Fidelis Care - New York and the Westbury Salvation Army to distribute information to community members about how to access healthcare services and health insurance. Our PICHC program also attended and assisted pregnant individuals with getting essential items like car seats, diapers and more.

More information on Continuity of Care that we referenced from our previous Case Success: Many major health insurance policies contain a process laid out for when one of your ongoing providers drops out of the insurance carriers network. This provides short-term or bridged coverage for you to continuing seeing your provider while you locate a new provider that is still in-network. This is useful as patients who are under an active course of treatment (i.e. pregnant, undergoing chemo/radiation, seeing a psychiatrist) so as to not disrupt the treatment and cause adverse health outcomes. OmniSalud (Connect for Health Colorado) is a program that allows low-income undocumented Coloradans and DACA recipients to enroll in affordable health insurance plans with financial assistance, it reached 11,000 enrollments in 2 days, which caps out enrollment in a Colorado Option plan: “SilverEnchanced Saving”. The savings were financial assistance for those enrolled in the program, which just started last year. Last year's program saw 10,000 people hit the enrollment cap in a month, with it hitting an expanded cap in two days the need for affordable care in a community, larger than 200,000, statewide is clear. https://lnkd.in/gh5z5xGg
