We performed an analysis on 800,000 social determinants of health assessments of our own patients and members. Although there is some variation by community, the top five needs across our footprint are: financial resources; health literacy; housing instability; social connections and safety. The study, published in JAMA, highlights the need to include individual and local data when building plans to address social drivers of health. We believe health systems, employers and government agencies should expand individual SDoH assessments to better inform care, program design and investments.
