Recognising and supporting the second victim: a valuable initiative by the The Royal College of Surgeons of England In the intricacy of patient care, a poignant reality often goes unnoticed: when adverse events occur, it isn't just the patient who suffers. The healthcare professionals involved, from doctors and nurses to technicians and management staff, can become the "second victims" — those deeply affected by the unintentional harm to their patients. Though trained to cope with high-pressure situations, healthcare workers are not immune to emotions of guilt, shame, and self-doubt. Support is needed to navigate these difficult emotions. When left unaddressed, the trauma can lead to burnout, depression, and even compromise future patient care. 🌟 We need to foster a culture that encourages open dialogue about adverse events, without judgment, and to establish a "just and restorative culture." 🌟 Support systems, like debriefing sessions and counselling services, should be readily available. 🌟 We need a robust process that supports all affected and promotes organisational learning. We know that adverse events happen in surgery, but over 42% of surgeons don’t talk to anyone, and the impact of these hidden stories is huge, for surgeons, teams, patients, and organisations. As a psychologist working with surgeons and other health professionals in the NHS, I feel strongly that resilience and wellbeing are much more helpfully seen as being fostered by strong, supportive connections between us, not just within us, and that the culture we are in shapes how colleagues feel and cope after adverse events. We need to ensure that no one is on their own when things go wrong in the operating theatre. That our surgical colleagues have proactive, accessible, compassionate support with peers who know the surgical context and challenges, alongside access to wider avenues of support. It’s therefore been a real pleasure to spend the day with Kevin Turner, Urology Consultant, Maddy Grenville-Harris, Psychologist, alongside surgeons from across the country discussing our new The Royal College of Surgeons of England SUPPORT collaborative (SUrgeon Peer-led Post-incident Response Teams) which is being launched in December. This is unashamedly an improvement collaborative! A process of learning together about how to best support colleagues. ACGME – the Accreditation Council of Graduate Medical Education – decides the minimum procedure requirements to graduate residency. It is the responsibility of training programs to have enough faculty and case volumes for residents to "train" effectively. Otherwise, the program will lose their training certification and fold.
