Case Report: Computed Fluid Dynamics Analysis for SVC-to-RPA Anastomosis With Antegrade Pulmonary Flow We would like to introduce a case report by Dr. Sumitomo and colleagues at Keio University. A 1-year-old boy was diagnosed with ventricular septal defect, atrial septal defect, hypoplastic tricuspid valve, and a small right ventricle with 50% of normal size. He underwent surgery including superior vena cava to right pulmonary artery anastomosis, closure of ventricular and atrial septal defects, and pulmonary artery banding. 🌟 **Pioneering the Future of Medicine** 🌟 Over the years, the field of biologic procedures has made incredible strides, helping to enhance the biology of the surgeries we perform. While I always exhaust every non-operative method first, some conditions ultimately require surgery. Yesterday, I performed a revision rotator cuff repair on a challenging, chronically torn, retracted rotator cuff, which are the stabilizing tendons around the shoulder. Not only did I repair it, but I also used a biologic patch, which acts like “topsoil”. In addition, we often use bone marrow cells, which function like “grass seed”, PRP (Platelet Rich Plasma) as a natural “fertilizer”, and laser therapy, akin to “sunlight”. (I love analogies!😀) When I first started performing arthroscopic rotator cuff repairs 26 years ago, I was the first in my community, and it was considered “not standard care”, I was told “you should not be doing arthroscopic rotator cuff repairs“, and it wasn’t even covered by most insurances. Fast forward 26 years, and now arthroscopic rotator cuff repairs are the standard of care. My hope for the future is that biologic procedures like BMAC and PRP, and many more to come, will follow the same path. There will always be naysayers, but there will also always be new wave pioneers, on the cutting edge of that wave, pushing the boundaries of what we know and advancing medicine to the next frontier. As a big Star Trek fan, I dream of a day when we have something like a Tricorder to diagnose and treat without any surgery or invasiveness.
