Fellows of integrative medicine join our two-year Fellowship after years of medical school, residency, and practice. We see cohorts of nurses who have dedicated decades to healthcare in our Integrative Health and Lifestyle certification program. Alumni of our Integrative Health Coach program include allied healthcare professionals with years in the field. The common thread that connects these healthcare heroes is a dedication to the pursuit of good medicine and health promotion. It's a valuable opportunity for learning and gaining insights into the practical aspects of a medical specialty. At our Community Care Center board meeting last week, Dr. Baldwin Smith, retired pediatric neurologist, shared a recent article from JAMA "Medicine-Both a Science (Care) and an Art (CARE)" (Strange TJ, Castellanos MR. Medicine-Both a Science (Care) and an Art (CARE). JAMA. 2024 Apr 23;331(16):1357-1358. doi: 10.1001/jama.2024.2508. PMID: 38568598). The authors emphasize that with technological advancements transforming medical care, "the timeless art of medicine (compassion and caring) remains a foundational aspect of patient-centered health care" (Trzeciak & Mazzarelli, 2019). The qualities of compassion, assistance, active listening, respect, and empathy "not only contribute to better patient outcomes and increased satisfaction, but also play a critical role in improving overall well-being" (Malenfant et al, 2022). CARE (compassion, assistance, respect, and empathy) - an acronym to guide all those providing medical care in a fast-paced, rapidly changing world.

As a young, early career physician drawn to improve systems, engaged in activism, I naively volunteered my time to advocate and to protest and to push for reform. Looking back, a younger me likely was a bit of an “ok boomer” mindset, assuming the old guard was just too staid. Now I see what the seemingly “change resistant” types were warning about and why they were against the “reform.” Business types, using buzzwords about centering patients, took over healthcare, removed clinicians from healthcare leadership, drove consolidation, drove up prices, drove down access, drove out patients. Outcomes went down. CEO pay went up. These are the outcomes that the business of healthcare delivers. We need a radical restructuring again, to keep clinicians and to get back access patients need.
