Young people (aged 10-24) represent 20% of the UK population. A Third of those aged 16 to 24 have long-term mental and physical health challenges and many more experience their initial presentation with a chronic health problem during their adolescent and young adult years. Yet it is well recognised that there are inconsistencies in the capabilities of our health system to deliver developmentally appropriate care, which is essential to improve long term health and psychosocial outcomes for this age group. This was evidenced in the recent NCEPOD Report (accessed here: https://lnkd.in/eUSvafDh ) One of the main key findings in this report is that developmentally appropriate care and transition services remain fragmented and inadequate for many young people. Many thanks to our physician trainees for their invaluable contribution in completing this survey (it should not take more than 2mn) to help us improve and standardise training in developmentally appropriate care and meet the needs of young people.

MEDICAL COUNCIL INVESTIGATIONS: THE IMPACT ON DOCTORS A Medical Protection Society survey has shown the negative impact of a Medical Council complaint on doctors and are seeking changes to the system. Of course there needs to be a mechanism to manage complaints effectively. If a letter of complaint arrives on a doctors desk, so much time can be spent trying to decide what to do, panicking about your future even catastrophising. In these circumstances the trauma can lead to acute stress. For some people that becomes a chronic stress which can impact overall health. By having a way to handover the complaint management to a professional such as Medical Protection, you can then focus on your mental and physical fallout from it. As an MPS member you can avail of counselling for yourself. Other resources may be EAP through work or private therapy. If you embark on private therapy ensure the person is appropriately accredited. In the realist review "Engaging family caregivers and health system partners in exploring how multi-level contexts in primary care practices affect case management functions and outcomes of patients and family caregivers at end of life" the critical community-based resources identified by patients and families dealing with end-of-life are: 1) having a healthcare professional or assistant trained in end-of-life care, 2) being able to access someone trained in end-of-life care when patients transition home after discharge, 3) help with coordinating the available services and supports, 4) having resources available to help caregivers cope with stress and care for the patient, and 5) providing extra physical and psychological support for patients living alone. Professor Donald Berwick has studied the management of healthcare systems for four decades. He is the founder of IHI, is Professor of Health Policy and Management at Harvard School of Public Health and has been a practicing Consultant in Paediatrics. He has a deep understanding of evidence based medicine, comparative research and of medicine on the shop floor (he has a very long bio including a knighthood for his work in the UK). We need BOTH individual skill and systemic leadership and structural change for healthcare to improve, for patients to receive better care. Whether we call it civility, professionalism, kindness, compassion, care, respect it all comes down to the golden rule... Treat others as you want them to treat you, or your best friend, or your mum. We can’t decide later when we are in the heat of the fire, it’s too late then. Decide now. Begin with the active intention to be kind – give each other the benefit of the doubt, recognise the other person’s humanity, understand they are depleted. Invite them into the contagious impact of kindness and keep inviting them. If they have been hurt or mistreated, if they are burnt out or cynical you might need to invite them more than once... We all want to be seen and heard, to belong. It starts with open hearted kindness. As the Dali Lama says “Be kind whenever possible.
