Systemic sclerosis interstitial lung disease: unmet needs and potential solutions - Systemic sclerosis (SSc) is a rare systemic autoimmune disease characterized by high clinical heterogeneity, in which interstitial lung disease (ILD) is one of the main causes of morbidity and mortality. - Despite many advances in the understanding of pathogenetic mechanisms and clinical definition, SSc–ILD management is still associated with several unmet needs. - Discovery of new therapeutic targets and specific diagnostic and prognostic markers will help optimize the management of SSc–ILD. - More refined stratification of patients on the basis of clinical features, molecular signatures, identification of subpopulations with distinct clinical trajectories and implementation of outcome measures for future clinical trials could also improve therapeutic management strategies, helping to avoid poor outcomes related to lung involvement. https://lnkd.in/dKzu3MVg #autoimmunity #scleroderma #autoimmunedisease #lungdisease

The fibrosis 4 (FIB4) index is a liver fibrosis biomarker that is a potential alternative to liver biopsy for diagnosing and managing liver disease. A simple non-invasive score (FIB-4) was developed using tree routine laboratory tests (ALT, AST, platelets count and age) for the detection of fibrosis in patients. Specifically, a value of FIB-4 below 1.30 is considered as low risk for advanced fibrosis; a value of FIB-4 over 2.67 is considered as high risk for advanced fibrosis; and FIB-4 values between 1.30 and 2.67 are considered as intermediate risk of advanced fibrosis.

Peripheral arterial disease is a chronic vascular disease that affects millions of people worldwide. Critical Limb Ischemia (CLI), its most severe stage, puts patients at an increased risk of cardiovascular events, amputation, and death. CLI is both common and deadly, and more incident cases die over 5 years after diagnosis than with any type of Cancer, except for lung Cancer. A recent study comparing incident cases of CLI and 22 types of cancer in the United States found that the diseases responsible for the most deaths over 5 years were lung cancer (192,000) followed by CLI (58,000). CLI is prevalent in 12% of the USA population and is an underrecognized major threat to public health. Screening the population with Ankle Brachial Index (ABI) is the first step in Preventive Interventional Radiology. Early detection and intervention can help reduce the risk of cardiovascular events, amputation, and death. We're reporting some of the most significant break throughs in hematology at ASH2023 in San Diego. While the focus has largely been on hematological cancers, a groundbreaking development has emerged for patients with life-threatening autoimmune diseases. Aaron Gerds, the chair of the ASH2023 opening press briefing, couldn't find a 🤯 big enough to express his amazement at Fabian Muellers presentation. Mueller showcased how CD19 CAR T-cell therapy has managed to completely control B-cell driven autoimmune diseases and is well-tolerated, all without the need for additional disease-related therapeutics. Mueller's study, involving 15 patients who received CAR T cell therapy, demonstrated significant and life-changing outcomes. Highlighting the profound impact, he shared a story of their first patient in the trial, a 21-year-old woman with Lupus, once with only 4-6 weeks life expectancy due to her severe disease. She is now jogging five days a week. This incredible innovation will open up a new era of treatment possibilities for patients with autoimmune diseases. Both myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) and long Covid (LC) share many clinical similarities. This review presents recent findings in ME/CFS research focussing on proposed disease pathologies shared with LC. Understanding these disease pathologies and how they influence each other is key to developing effective therapeutics and diagnostic tests. Given that ME/CFS typically has a longer disease duration compared with LC, with symptoms and pathologies evolving over time, ME/CFS may provide insights into the future progression of LC. #chronicfatiguesyndrome #longcovid

Conventional two-dimensional (2D) cell models and animal models cannot mimic the human-specific properties of the lungs. In the past decade, human organ-on-a-chip (OOC) platforms—including lung-on-a-chip (LOC)—have emerged rapidly, with the ability to reproduce the in vivo features of organs or tissues based on their three-dimensional (3D) structures. Furthermore, the integration of biosensors in the chip allows researchers to monitor various parameters related to disease development and drug efficacy. In this review, we illustrate the biosensor-based LOC modeling, further discussing the future challenges as well as perspectives in integrating biosensors in OOC platforms.
