Many will think this was an accident. Unfortunately, it was a failure. There were many controls in place and presumably a back-up plan for every back-up plan, but this massive ship still managed to collide with a bridge just outside of one of the continent's largest ports. Can you deduce what the root cause was? #failureanalysis #quality #plan #baltimore #tragic https://shorturl.at/wyST5

The Francis Scott Key Bridge in #Baltimore, which carries an average of over 12 million vehicles a year, collapsed on March 26 after a container #ship lost power and struck a pylon. The remains of the #bridge now lie in water around 50 feet deep, blocking access from the port to #Chesapeake Bay. There are eight other bridges in the US that are vulnerable to a similar incident as the Key Bridge, as they are the only ones that allow the passage of ships of such a size and the failure of just one steel node could knock out the whole bridge. About 17,000 US #bridges lack structural redundancy, exposing US economic activity to acute emergency situations and major #supplychain and #logistics disruptions.
