Our new article has already been published! Assessing the Effectiveness of qSOFA and SIRS Scoring Systems:t a Single-Center Pavlina Peneva, Yana Bocheva, Silviya P. Nikolova Abstract In the context of post-2016 redefinitions of sepsis and septic shock, this prospective, non-interventional single-center clinical trial was carried out at St. Marina University Hospital, Varna, Bulgaria. The study encompassed 87 participants, approximately 45 of whom presented with sepsis and septic shock. Employing logistic regression, the study aimed to assess the predictive value of the Systemic Inflammatory Response Syndrome (SIRS) and quick Sequential Organ Failure Assessment (qSOFA) criteria in sepsis. Additionally, sensitivity and specificity were evaluated using ROC curve analysis. The findings revealed the substantial predictive potential of both SIRS and qSOFA for septic patient mortality. Notably, the qSOFA score exhibited a significantly higher likelihood (2.581 times, p = 0.0001, 95% CI 1.557 - 4.279) of predicting mortality compared to the SIRS scale (2.050-fold, p = 0.004, 95% CI 1.255 - 3.349). The study contributes to guiding the enhancement of patient care strategies, effortlessly aligning with the core tenets of evidence-based nursing practices.

I recently PGx tested one of my nursing home patients. He is an 81 year old male with a significant history of behavior problems. His results came back with several gene-drug interactions. Metoprolol - CYP2D6 IM Tramadol - CYP2D6 IM Pravastatin - SLC01B1 decreased function I am wondering if the decreased metabolism of metoprolol was contributing to CNS effects. His pain was not well controlled with tramadol. He may have been experiencing muscle pain from the statin. Were all of these contributing to behaviors? He was recently switched from Olanzapine to Risperidone. Preliminary studies suggest a favorable response to Risperidone for patients homozygous for T allele of DrD2 variant rs1799978. He was also taking Pramipexole 1.5mg BID + PRN for restless legs. Opposing actions of pramipexole and risperidone decreases the efficacy of both medications. I recommended checking iron studies and gabapentin as an alternative for restless legs. Also interesting, he has a history of blood clots and has the factor V leiden variant. He also has dementia and has two copies of the APOE ε4/ε4 phenotype. I can see why he was having significant behaviors. Scholarly Project: Sub-anesthetic Ketamine's Role in Improving Patient Outcomes during a Sickle Cell Vaso-Occlusive Crisis "You are your own best thing" ~ Toni Morrison Standing on business for mine, always. ✊🏾🖤🤎 It was imperative for me to do my senior research project on a topic that can potentially impact the health of black people. One of the main reasons I am in healthcare is help tether the bridge that closes the health equity gap in this country. Sickle cell disease may only affect 100,00 Americans, but it is the most common inherited hematologic disorder in the country. The disease mostly affects African Americans and those of Sub-Saharan African descent. For perspective about 1 and 13 African American babies are born with sickle cell trait. The primary morbidity or the disease is pain (acute or chronic). It is not uncommon for these patients to be prescribed opioids at home to manage their pain. During vaso-occlusive crisis (VOC) however the pain experienced is more intense and refractory to opioids. This leads to ED visits for VOC. Sometimes ED providers have a negative attitude when treating these patients with more opioids. This contributes to under and poorly treated pain in this population. Ketamine is a non-opioid anesthetic that is a great drug to be introduced in this conversation of treating SCD-VOC. SCD patients also experience opioid induced hyperalgesia (OIH) which is a maladaptive response of the CNS to chronic pain or under treated acute pain. On a cellular level OIH has an up-regulation of NMDA receptors and a down regulation of Mu receptors. And guess what, Ketamine is a NMDA antagonist! Making it a great contender for refractory pain/OIH/SCD-VOC. There is still much research to be done but Ketamine deserves a seat at the table to help this marginalized patient population achieve better analgesia. #blackwomen #futurecrna #drexeluniversity
