The situation in the US is a clear signal: we need to focus on primary care. It's crucial that other countries take notice and strengthen their own healthcare systems at the grassroots level. By supporting more primary care providers, we can improve health outcomes and increase life expectancies. Let's push for policies that back this approach, inspire medical students to enter primary care, and invest in a health system that values patient care over profit. Identify how older adults are at risk for disproportionate harm in healthcare encounters and the impact this has on health equity. 2. Discuss the individual components and inter-relationship of the 4 M's framework: “What Matters” to the patient and family, “Medications,” “Mentation,” and “Mobility". 3. Evaluate the impact of incorporating the 4Ms assessment and care strategies in the outpatient and inpatient setting through a case-study. 4. Describe current and anticipated national policy changes related to the 4Ms as a driver of high-quality, equitable care for older adults. 5. Apply Age-Friendly Health System care practices at local institutions, both internal and external to Sutter, to promote collaboration towards Age-Friendly communities.

The 2023 Primary Care Collaborative Evidence Report, Health is Primary: Charting a Path to Equity and Sustainability, looks at factors driving the downward trends in primary care: fewer clinicians practicing primary care, fewer available primary care appointments for patients, lower spending on primary care as a share of total spending, and fewer adults reporting that they have a usual source of primary care. Expert panel to guide ‘incredibly broad’ NSW State Health Enquiry. https://lnkd.in/gDMz6YHU A great article in the The Sydney Morning Herald worth reading carefully. Those of us on the frontline of healthcare are putting our trust in Commissioner Richard Beasley SC and his Special Commission of Enquiry established by NSW Premier Chris Minns. The mess we are in was guided by experts, consultants and bureaucrats… I am not convinced we need more input from them. Einsteins definition of stupidity is doing the same thing repeatedly and expect a different result. Dr Michael Bonning of the Australian Medical Association (NSW) who is a practising GP mentions they might need some input from those who actually are Nurses, Doctors, Paramedics, Technicians, and frontline administrative support. This would be the understatement of 2024. General Practitioners were marginalised in the COVID response by Scott Morrison and his ‘Centralised’ Government of that day. The public were, in my opinion, led to believe Government, Experts, Health Department and Health district Executives knew what was best for the public. The result of this failed policy is here for all to see- when patients and families use hospitals as primary care. It cannot work. In New South Wales the Hospital system is bloated by too many managers who have either no clinical experience or any recency of experience. Decisions which can be best made at the clinical coalface are moved up a tree into which they are slower, less appropriate and more expensive. We will submit that the bloat and deadwood in Healthcare administration needs substantial pruning. The estimated cost of this enquiry will be a substantial saving in the longer term if it fulfils its terms of reference. Richard Beasley SC is at the top of his profession having got there without spending a life in administration and management. This could work!
