Landstuhl Regional Medical Center team leads medical readiness exercise in Ghana A 25-member team composed of U.S. Army and U.S. Air Force medical, veterinary, and dental personnel from Landstuhl Regional Medical Center (LRMC), Dental Health Activity-Rhineland Pfalz, Public Health Command Europe, the USAF 86th Medical Squadron, and the North Dakota National Guard, participated in a three-week medical readiness exercise, or MEDREX, together with members of the Ghana Armed Forces, May 12 - 31, 2024 at the 37th Military Hospital in Accra, Ghana.

With the most recent fatality due to High altitude decompression sickness, it's great to see 345th Medical Group and the 168th working up their austere hyperbaric capabilities using the Hyperlite 1. Portable hyperbarics permit a transfer under pressure up to altitudes of 17,999 ft unpressurized; allowing for rapid transport of DCI casualties to a mulit-place facility while still conducting HBOT. In the case of the Hyperlite 1, it's maximum operating depth is 23 MSW permitting operators to conduct TT5, TT6 and TT6 Extended. Rapid HBOT is the gold standard of care for DCI casualties, delay in treatment only serves to increase the risk of permanent injury or even death. IMPROVING COMBAT CASUALTY SIM OPTIONS Good things happening at Ft Cabazos, TX (Formerly Fort Hood) – The Fort Cavazos Medical Simulation Training Center, along with medics from the III Armored Crops and U.S. Army Medical Test and Evaluation Activity agency, are working on high fidelity female mannequins to address discrepancies in combat casualty care and prepare for future conflicts. Rates of service member death for women have been 2.5% higher in Operation Iraqi Freedom and 18.9% higher in Operation Enduring Freedom. Potential reasons for this gap are documented slower performance by male medics when assessing female casualties, previous mannequins being too small to accurately replicate female servicemembers, and lack of understanding of female physiology. Beijing’s ability to successfully treat wounded soldiers and sailors would likely be a factor when deciding to enter into most conflicts. A People’s Liberation Army (#PLA) source has stated that China could expect 120,000 casualties in an amphibious invasion of Taiwan. Even such a large projected number of casualties is unlikely to deter the PLA from entering into a conflict when it involves the core issue of Taiwan. As such, the PLA is showing a focus on care for the wounded. Authoritative PLA publications provide detailed information on planned battlefield medical support and evacuation of wounded from the company level back to field hospitals and fixed PLA hospitals.[i]

Leaders all know that readiness is a critical driver in mission success. This is true in many fields and most certainly in healthcare and defense. We talk about it all the time, but most effective for inspiring buy-in, is sharing evocative examples of why readiness is so important and how it makes a difference. The article I've attached here is a perfect example. When the Navy was called upon by our civil partners to execute a search and rescue operation in the Eastern Sierra Mountains, a hard landing by the helicopter resulted in injuries, and no way off the mountain for the rescue team. I was asked to provide hospital corpsmen to support the security team who were deploying in hours, not days. After a fast review of mission requirements and logistics, two cold weather, mountain trained corpsmen jumped into action. Over the course of almost a month, five of my Navy medicine hospital corpsmen kept a team of security personnel safe while they camped and traversed between altitudes of 6000 and 9500 feet in cold, snowy conditions on a daily 12 mile roundtrip hike. We don't always have days or weeks to prepare for action. When we only have hours to deploy, our team needs ready skills, ready gear, ready mental and physical resilience, and ready support staff to ensure mission success. Is readiness important? It’s international Clinical Trials Day today! A brief introduction and why modern clinical trial is celebrated globally every May 20. This is in recognition of the day in 1747 that James Lind, a ship’s surgeon in the British Royal Navy, started what is now considered to be the first #randomized clinical trial. In this case, it was to study the effects of different treatments on scurvy, a disease caused by a deficiency of vitamin C, and a major scourge for sailors on long voyage. Lind selected 12 sailors suffering from #scurvy and divided them into six pairs, each receiving a different treatment over the course of a week. The treatments included cider, vinegar, sea-water, a mixture of garlic and mustard, barley water, and citrus fruits. The sailors who consumed citrus fruits recovered, demonstrating the effectiveness of citrus in curing scurvy. Today, clinical trials are used to test the safety and effectiveness of all kinds of #treatments and health improvement solutions. #clinicaltrialsday #clinicalresearch
